Contamination污染 Any situation where a customers product is out of contract specification and can not be used as intended or is reduced in value, or where any asset is filled with the incorrect product as a result of a Vopak act or failure to act.在任何情况下客户的产品超出了的，而不能按原要求使用或降低了使用要求。或者由于孚宝错误操作而充装了错误的产品。 Fatality死亡 A work related accident leading to death within one year after its occurrence. 由于与工作相关事故在发生后一年内而导致的死亡。 Fire火灾 Any unintended fire, including flames, smoke or smoldering that occurs within any part of the terminal or a building.在库区或建筑内的任何地方的非正常失火、烟雾或不完全燃烧定义为火灾 First Aid Case急救 Any work related injury requiring one-time treatment or subsequent observation of minor injuries by a person trained in first aid.对于任何与工作相关伤害且需要由受过专业培训的人员进行的一次性处理或进一步观察的轻微的伤害叫急救。     Lost Time Injury损工伤害 Any work related injury that causes the persons to be absent from work for at least one shift because he/she is unable to perform any duties. (not including the shift when the injury occurred or any days the person would not have worked). 任何由于工作造成的伤害并造成至少缺席一个班次不能做任何工作（不包括伤害发生时的班次也不包括休息日） If the person is injured when travelling to their normal place of work this is not an LTI, but if the person is injured while travelling as part of their work then this is an LTI职工在上班途中造成伤害不是LTI，但是旅途是他工作的一部分造成的伤害是LTI。 Lost Time Injury Rate损工伤害率 The number of lost time injuries per 1000000 available working hours百万工作小时发生的损工伤害数量。 Medical Treatment Case药物处理 Any work related injury which requires treatment by a physician or other licensed health care professional other than First Aid. This does not include visits to a physician or other licensed health care professional solely for observation, counseling or diagnostic procedures.任何由于工作造成的伤害必须由医生或具有资质的专业人员进行处理，除了急救以外的伤害。这不包括去看医生或专业医师进行咨询、观察和诊断。 Near Miss险兆事故 A sudden unintended event which could have caused, if it had progressed to the next stage, injury to any person, interruption of work, damage, spillage or contamination. 产生突发不可预见的事件，其事件进一步发生会造成人员的伤害、工作中断、财产损失、泄漏或污染的事故 Permit violation违规事件 Any formal (written) permit violation notified by qualified authorities. For example a fine or penalty or an official letter from the authorities to confirm a permit violation by the company. Each separate occasion counts as a permit violation even if they all relate to a single incident. 任何由有资质的政府部门发出的正式的（书面）的违规处罚。例如来自政府认可部门开具的罚款和书面处罚。即使违规处罚都是由于同一个事件，每一个处罚也要单独统计。 Restricted Work Case工作受限事件 Any work related injury leading to a person being physically or mentally unable to perform all or any part of their normal job duties or any part of their normal workday or shift, not including the day or shift when the incident occurred or any days the person would not have worked. The person is assigned to alternative work for the part he is unable to perform the normal job duties.任何由于工作造成的伤害导致员工无论从身体上还是从精神上不能够全部或部分完成他的正常工作任务，或者不能够按照正常时间上下班，不包括事故发生时的当天或当班。可以安排他做另外的工作，但他不能完成其工作职责内的工作。     Spill/Leak泄漏和溢出 Any escape of product or waste related to a product transfer, storage or cleaning operations, regardless of the hazard of product, and regardless of whether or not the release is in a contained area.
