宠物乐园寄养宠物细则 寵物樂園寄養寵物協議細則 -「本會」是指 「寵物樂園.」。 - 「寄養寵物」是指任何由本會同意接受其寄養之寵物。 (1.) 凡委託本會 寄養寵物人仕,必須為寄養寵物的物主或其書面授權人。 (2.) 所有寄養於本會的狗隻,其年齡必須不少於四個月大,而屆滿十歲或超過十歲 任何寵物,則該動物必須於入住前一星期由註冊獸醫檢驗及經註冊獸醫書面同 意後,才能獲准寄養本會。 (3.) 所有寄養於本會之狗隻,必須持有有效預防針咭正本。證明狗隻已接受完全性 的G6混合防疫注射,即基本期首次和至少一次加強劑之防疫注射及每年之加 強劑防疫注射,,五個月大或以上的狗隻必須接受過瘋狗症預防注射及已植入確 認身份的晶片。 (4.) 若寄養寵物只接受了基本期之首次和至少一次的防疫注射,那防疫針的最後一 次注射,則必須在寄養之前七天或以上完成。每年一次的防疫加強注射若已過 期三個月者,其防疫注射記錄將被視為失效。 (5.) 客人必須於寄養前出示有效之防疫注射記錄正本,若寵物在入住時,未能出示 有效之防疫注射記錄正本,本會恕不接受該寵物的寄養,而有關的寄養寵物訂 金及有關的費用將不會退還。 (6.) 本會擁有絕對酌情權在任何情況下或不提供任何理由,拒絕接受寄養任何寵 物,兇猛或不健康的寵物均不被接受。 (7.) 凡寄養於本會的寵物,必須作定期殺蚤,本會亦建議客人替寄養寵物在寄養期 起碼24小時前做預防心絲蟲之措施,假如客人不選擇以上建議,而其寄養寵物 期間因心絲蟲染上任何疾病及導致傷亡,後果一概由客人負責。 (8.) 本會寄養及額外服務費用,將根據本會最新之收費計算,寄養費用只包括住 宿、基本健康檢查、一般房舍清潔、每天由本會安排的戶外活動和本會提供之 指定膳食,並不包括其他服務如特別糧食、美容、獸醫診治、醫療護理、額外 散步、接送等。 (9.) 所有寄養寵物在寄養期內,如遇有身體不適,本會有權酌情決定該寵物是否需 要獸醫診治,及跟進藥物服用等,獸醫診治及藥物費用、醫療測試費用、必須 的手術費用、留醫費用、本會的護理服務費用及車費等,其主人或代理人必須 於寵物寄養期滿時即時繳清有關費用。 (10.) 若客人在其寵物寄養期間增加額外服務,有關費用須在其寄養寵物離開本 公司前繳清。若客人在寄養期屆滿後仍未繳清費用,本會會根據“會員申請表” 上的客人地址,寄發久款通知,如七日內未能收到欠款,本會有權將動物送交 別人領養,放棄或毀滅,而領養所得金錢則當作賠償本會之費用。 (11.) 寄養寵物只可由其主人或其主人書面授權之代理人方可領回,而寄養寵物 離開寄養中心前,其主人或代理人須簽署客戶聲明,以確實其寵物在離開時之 身體情況。 (12.) 接送服務乃本會的一項額外服務,如遇上任何預料之外的情況,例如,交 通或天氣問題等,導致延誤接送服務,本會將不負任何責任,而寄養寵物因此 而逗留本會之額外日子,主人亦應支付所需之寄養費用。 (13.) 本會有權隨時調整寵物之寄養費用,而無須事先給予通知。 (14.) 本會會盡最大努力預防被寄養寵物之逃走、損傷、疾病(如皮膚病或腸胃不 適等) 甚至死亡,如上述事故不幸發生,本會概不負責。 (15.) 本會保留權利隨時修改、取代、刪除寄養條例所載的任何或任何或全部條 款及條件,或訂立新條款及條件而無需預先通知。 (中文)___________________ 本人:(英文)___________________身份證號碼: _______________,清楚明白上述條款,並無異議 會員簽署,,,,,,,,,,,,,, 日期,,,,,,,,,,, Boarding Conditions (1) Owner should show his ID card and address when being asked by the society staff (including the driver). (2) The society has absolute discretion to accept or reject any animal for boarding. (3) Owner is required to provide the pet’s medical history, veterinary’s data and the information of urgently contact person. (4) All dogs must have been fully vaccinated and have received regular annual boosters against Distemper, Provirus, and Rabies. The person wishing to board an animal is responsible for ensuring that vaccination records are available and adequate before the animal is presented at the kennels. (5) In the boarding period, any charge for veterinary treatment and for medicine is a responsibility to the owner. (6) The boarding charges, which are subject to change without notice, shall only cover board and feeding are exclusive of any charge for veterinary treatment and for medicines, which may be veterinary treatment is required during the boarding period, the decision as to whether veterinary treatment is required and the type of treatment to be adopted, including surgical operations, shall be entirely at the discretion of the veterinary surgeons. (7) Animals will be fed a standard balanced diet as directed by the veterinary surgeon. Owner can supplies a special diet prescribed by a veterinary surgeon. (8) Pick up & Delivery Service is an extra service. Any unexpected conditions such as traffic accident or unstable weather lead to the delay; the society will not bear any responsibility. And the owner still has to pay for the extra boarding fee under these circumstances. (9) The society will only arrange room according to the boarding period before the animal check in. For the extra boarding days, the society has to consider the situation. The society has absolute discretion to require the animal to leave if rooms are not available. (10) If any charge due from the owner, or the person who deposited the animal on behalf of the owner, are not paid within seven days after notice has been sent to the owner, recorded at the members application for, the society may at it absolute discretion, whether during or after the boarding period, give away, rehome, destroy or otherwise dispose of the animal and will retain the proportion of the proceeds to recover the outstanding charges due to the society. (11) The owner, the person who deposited the animal, or an agent of the owner duty authorized in writing, by the owner are the only persons authorized collect a boarding animal. (12) Any animal accepted shall be boarding with the society at the sole risk of the owner of the animal and the society shall not under any circumstances what so ever be liable in the event that the animal depreciates in value, sustains injury, dies, ill, lost or escapes. (13) The society has absolute discretion to amend, replace and/or make alteration to these “Boarding Conditions” at any time without prior notice.
