童病因不明的急性发热诊断处理指南 (解读版 ) 0, 本刊将
从 2009年第 1期起连续 3期全文刊载, 以飧读者。
1 发热的定义和体温的测量
1. 1 发热 正常体温在 1天中有波动,并随不同年龄和不
同体温测量方法 (部位和工具 )而有所不同。在研究文献
中, 有几种不同的发热定义, 本文采用的发热定义是指: 体
温升高超出 1天中正常体温波动的上限 [ 1]。
目前大多采用直肠温度 \ 38 e 定义为发热。高热临
界值大多设定为 39e 或 39. 5e ,有的甚至采用 40e 。对于
0~ 3个月的婴儿正常直肠温度为 ( 37. 5 ? 0. 3) e , 直肠温
度高于正常值 2个标准差 ( 38. 1e )则为发热 [ 2] , 对大龄儿
童而言, 正常直肠温度为 37. 5e [ 3]。
1. 2 体温测量 儿童体温测量可通过不同部位、应用不同
温度计进行测量。
体温测量部位: 口腔、直肠、腋下、额部和耳道。
体温测量仪器: 玻璃水银体温计 (图 1A )、电子体温计
(图 1B )、红外线电子耳道体温计 (图 1C )、红外线测温仪
(图 1D )和化学标点 (相变 ) [ chem ica l do t( phase-change) ]
测温额贴 (图 1E )。
玻璃水银体温计为传统的测量体温工具, 但因其易断
裂并发生水银泄漏 [4] ,现已不主张应用于婴幼儿 [ 4]。国外
仅作为发热诊断对照性研究的金标准, 国内目前大多采用
此方法测量体温。电子体温计具有测体温准确和快速的优
点,在发达国家逐渐取代玻璃水银体温计用于儿童测量体
温。红外线测温仪应用越来越频繁, 通过检测体内血管红
外辐射估测体内中心温度, 通过鼓膜和头皮颞动脉测温, 测
体温快速, 但价格较为昂贵。红外线电子耳道体温计测量
体温偏差较大。化学标点 (相变 )测温是通过化学反应的
颜色变化来判断体温变化, 制作成额贴, 国外有化学标点
(相变 )测温塑料胶棒,可反复使用。

临床问题 0~ 5岁儿童不同部位应用不同体温测量
工具所测体温的准确性如何?
不同部位测温的证据发现有 1篇系统综述 [3] ( Ò a)和
20项前瞻性研究 ( 2 项 Ñ b[ 5, 6] , 10 项 Ò [7~ 16] 和 8 项
Ó [ 17~ 24] ), 由于文献 [ 17~ 24]纳入研究对象的年龄为 0 ~ 14
岁, 研究地点和体温测量方法各异, 故无法进行 M eta分析
得出结论。
有 2项研究 [ 25, 26]应用玻璃水银体温计或电子体温计
检测 < 2岁患儿口腔或直肠体温 (Ò a),结果显示电子体温
计与玻璃水银体温计比较, 测量口腔和直肠体温的敏感度
分别为 10%和 63%。 [ 5岁儿童采用玻璃水银体温计口
腔测温可因位置不正、测量不准、温度计被咬断和疼痛, 而
致测温困难 [ 27]。口腔测温适用于大龄儿童和成人,结果准
确 [ 27] ;直肠测温适用于婴幼儿, 结果准确, 但患儿较难接
受, 且安全性不佳 [ 27]。专家共识认为 0~ 5岁儿童不推荐
通过口腔或直肠测温 (Ô )。
文献 [ 3, 28]表明儿童采用玻璃水银体温计在腋下所测温
度平均低于耳温 0. 5e 以上 ( 0. 09 ~ 1. 52e ), 平均较直肠
体温低 1e ,敏感度为 25% ~ 98%。而采用电子体温计在
腋下测温可较直肠体温低, 最多可达 2e [ 3]。而新生儿直
肠体温与腋下所测温度差值分别为 0. 09e ( 95% C I:
0. 06~ 0. 12e ) [ 13]、0. 3e [ 29]和 0. 2e [ 6], 平均为 0. 17e [ 3]。
腋下测温适用于新生儿,快速易行, 且结果准确 [ 25, 26]。
采用化学标点 (相变 )测温有 3 项前瞻性队列研
究 [ 15, 25, 30] (Ò ) ,分别为新生儿、3个月和 3岁的儿童 ,与玻
璃水银体温计腋下所测温度相比低 0. 32[ 30]和 0. 93e [25],
敏感度为 68% [ 15]和 92% [ 30]。
1项研究 [ 31]是关于红外线测温仪颞动脉测温的研究,
研究对象为 327名 < 2岁的婴幼儿, 发现红外线测温仪颞
动脉测温可检测出 81%的直肠体温\ 38. 0e 和 88%的直
肠体温\ 38. 3 e 的儿童 (Ó )。
4项评估化学标点 (相变 )额部测温与不同测温计在不
同部位测温比较的研究 ( 2项Ò [ 26, 32], 2项Ó [ 33, 34] ), 因研
究对象、对照方法及测温部位差异较大, 因此有限的证据仅
提示化学标点 (相变 )额部测温较玻璃水银测温计直肠体
温平均低 1. 2e [ 34] ; 化学标点 (相变 )额部测温适用于所有
患儿, 快速易操作,但结果欠准确 [ 35]。
有研究提示: 对各年龄段患儿而言, 红外线测温仪准确
性、一致性和敏感度较差 [ 36]。
有 2项系统综述 [ 27, 28] ( Ò )和 21项前瞻性研究 ( 2项
Ñ b[ 37, 38] , 8项Ò [ 8, 10, 13, 39~ 43]和 11项 Ó [ 44~ 54] ) 报道红外线
电子耳道测温的结果 ,其中系统综述 [28]纳入了 4 441名 0~
16岁的儿童, 汇总分析提示红外线电子耳道测温与其他部
位测温的平均温差在 0. 29 e ( 0. 74~ 1. 34e ), 并受年龄、
情绪、环境温度和设备等因素的影响。其敏感度和特异度
分别为 63. 7% ( 95% C I: 55. 6% ~ 71. 8% )和 95. 2%
( 93. 5% ~ 96. 9% ); 红外线电子耳道测温可有 30% ~
40%的发热患儿被漏诊 [ 55]。对 3个月以下婴儿应用红外
线电子耳道测温并不可靠, 且耳道放置温度计有一定困难。
临床问题 发热体温高度是否与疾病严重程度相关
有 12项前瞻性队列研究 [ 56~ 67] ( 9项 Ò a, 3项 Ò b)表
明,发热体温高度与严重疾病间存在相关性, 文献 [ 68]结
果表明:高热与严重细菌感染间存在相关性, 特别是当体
温 > 39e 时, 严重细菌感染危险性增加。发热的新生儿
中,发生严重细菌感染的危险性在 12% ~ 32% [ 69~ 71], 1~ 3
个月为 15% ~ 21% ,高于 3~ 36个月的婴幼儿 [72]。年龄越
小,发生严重细菌感染的危险性越大。当 < 3个月的婴儿
体温 > 38e 、3~ 6个月的婴儿体温 > 39e 即作为严重细菌
感染的高危因素之一。
临床问题 发热持续时间是否与疾病严重程度相关
前瞻性研究 [ 66, 73, 74]对发热持续时间与严重细菌感染
的关系进行研究。 1项研究 [ 74]表明发热 > 48 h发生严重
细菌感染的 OR为 3. 85 ( 95% C I: 1. 11 ~ 13. 3)。文献 [ 66]
提示在有严重细菌感染组的发热持续时间为 ( 26. 5 ?
41. 5) h, 明显长于非严重细菌感染组 ( 18. 6 ? 21. 7) h
(P < 0. 01)。而与发热时间 < 24 h组相比, > 48 h组严重
细菌感染的 OR为 1. 04 ( 95% CI: 0. 35~ 3. 12) [73]。文献
[ 66, 73]研究发现脓毒症时体温\ 39e 时间亦可较短。已
有的证据表明发热持续时间尚不作为预示严重疾病的危险
因素。
体温测量的建议
新生儿测体温应采用腋下电子体温计测体温 (Ò a)
1个月至 5岁儿童可采用腋下电子测温计或红外线测温仪
测温 (Ó )
化学标点 (相变 )测温 (额贴 )方法不可靠,不主张
采用 (Ò a, Ò b)
口腔、直肠采用电子测温计测体温最为经济有效 (Ò )
不能以发热体温高度预测发热病因及疾病严重程度 (Ò a)
发热持续时间不作为预示严重疾病的危险因素 (Ò b)

例, 预后良好,不被人们所重视。然而, 手足口病可以引起流行和爆发, 也可以出现重症病例,甚至导致死亡,严重威胁儿童
身体健康, 甚至造成社会恐慌。为提高和普及手足口病诊断和治疗的相关知识,中华医学会儿科急救学组副组长、湖南省
医学会副会长兼急诊专业委员会主任委员、湖南省儿童医院院长祝益民教授, 根据儿童手足口病救治经验, 主编了 5手足
