MU.n hills green, River afforestation increased, City Green, farmland and other greening works to ensure complete afforestation 均32.3岁。均已排除下鼻甲明显水肿、息肉样变和明显“桑椹样”肥大患者。 1.2 方法 所有患者根据鼻塞程度分为:严重鼻塞(+++):鼻塞严重,张口呼吸,伴头痛及闭塞性鼻音;中度鼻塞(++):介于重度和轻度之间;轻度鼻塞(+):间断性鼻塞;无鼻塞(-)。每位患者于术前、术后1周、半个月及3个月分别记录鼻塞情况,同时用自动前鼻测压计记录鼻腔阻力变化情况。 手术方式及方法:1%利多卡因于手术侧下鼻甲作浸润麻醉,于下鼻甲前端作类似于下鼻甲粘骨膜下切除术切口,但切口尽量靠下鼻甲前缘,自切口处用鼻骨膜分离器先分离下鼻甲内侧面的粘-骨膜,再分离下缘,使粘膜张力减小;然后再分离下鼻甲外侧面,分离深度直达下鼻甲后端;用下鼻甲剪沿下鼻甲骨根部剪断前端骨质,下鼻甲剪伸入深度一般不超过1 cm;用咬骨钳或筛窦钳取出下鼻甲前端骨质;然后根据下鼻甲后部肥大程度,酌情行下鼻甲后部外展骨折或下鼻甲后部骨质部分切除加外展骨折术。术毕,切口可缝合或不缝,鼻腔用凡士林纱条填塞,24,48 h取出。所有患者术后随访1年,观察症状复发情况。 2 结果 所有患者手术后反应较轻,无一例术后发生严重出血,术后鼻塞改善情况及鼻阻力变化见表1。 治疗下鼻甲肥大的新方法--双极低温射频消融 对于某些肥大的下鼻甲,使用鼻腔粘膜血管收缩剂及中西药物治疗,效果欠佳。除药物治疗外,改善鼻通气的方法还有下鼻甲部分切除术、冷冻、微波、激光及鼻息肉切割器部分切除等;但上述方法对下鼻甲粘膜损伤较大,术后疼痛,出血,结痂,干燥,鼻腔异味,感染,鼻腔粘连,不同程度影响鼻腔的功能。 目前鼻塞的手术治疗都是围绕下述3个方面来进行:1鼻腔内侧壁手术,即鼻中隔手术;2鼻瓣区手术;3鼻腔外侧壁手术。其中外侧壁手术主要是通过下鼻甲手术来完成,因为下鼻甲为一勃起组织,其肿胀或肥大能引起鼻塞及鼻阻力的明显改变。既往治疗下鼻甲肥大的方法,如冷冻、激光、微波、下鼻甲部分切除手术等均破坏了正常的鼻粘膜结构,影响鼻腔的生理功能。随着对鼻腔生理功能的逐步认识和重视,下鼻甲手术应向着“保护下鼻甲功能的同时将其缩小到最佳体积”的目标不断进取。 低温双极射频消融治疗系统的作用原理是使电极和组织间形成等离子薄层,层中离子被电场加速,并将能量传递给组织,在低温下(60oC―80oC)打开细胞间分子结合键,使靶组织中的细胞分解为碳水化合物和氧化物造成组织凝固性坏死,称为低温射频(不是热效应)。坏死细胞脱落或产生瘢痕收缩,达到靶组织体积减容的效果。 采用低温射频消融技术治疗下鼻甲肥大疗效显著,确切。具有以下优点:1) 微创伤性,整个治疗只需在下鼻甲前端刺一个小孔,最大浸透深度为2mm,对鼻甲粘膜的纤毛功能影响较小,对鼻甲周围正常组织损伤的可能性也较小。因此,术后很少有痂皮,鼻腔粘连等并发症发生的机率较低。同时,该治疗系统还具有自动控制功能,当组织变性时电阻增大到一定程度,系统会自动停止工作,并发出报警声。2) 精确度高,数字化的功率调节有利于选择合适的功率。在内窥镜辅助下,能充分而精确地缩小肥厚的鼻甲组织,对仍有肥厚的部分还可适当补充。3)止血效果好,术中出血一般为1-5ml,视野清晰,有利于操作的 -k of environmental protection: (a) in accordance with the ... ... MU.forestnt the barren hills green, River afforestation increased, City Green, farmland and other greening works to ensure complete af, mountain forests of" principle, to develop "governance focused piece, scale" afforestation programmes, and actively implemelined 准确。 鼻病时,为什么常常有鼻甲变大的现象? 10-25 20:55:42 临床上曾遇有患者因鼻阻塞,自己对着灯光照镜子,而发现鼻孔内“多出了一团红红的因”,十分惊恐,怀疑是否鼻内长了瘤子,要求耳鼻咽喉科医师给予进一步诊查。经详细检查,认定“这多出的红红的一团肉”,是人人鼻内都有的重要的组织结构,,下鼻甲,只不过该患者的下鼻甲比较大一些而已。 那么,究竟是什么原因促使鼻甲变得大起来了呢? 还是从鼻甲的解剖特点谈起。大家是否还有印象?在鼻腔的外侧壁存在着三个呈阶梯状排列的鼻甲。鼻甲是由鼻甲骨和包裹于其外的于层又厚又软又有弹性的粘膜所组成,这种粘膜富含毛细血管及小静脉,形如海绵状血窦。鼻甲粘膜海绵状血窦这种特殊的结构,便于通常情况下里收缩状的粘膜内小血管在短时间内随需要而快速扩张,对鼻的生理功能具有重要的调节作用。 鼻甲交大并不是一个孤立的疾病,也不是一个症状,而往往是耳鼻咽喉科医师在进行鼻专科检查时发现的,因此它只是一个体征。在三个鼻甲中以下鼻甲最大、最长、最靠近鼻孔,临床上以下鼻甲变大最为多见,其次为中鼻甲,上鼻甲变大较少见。下鼻甲变大可以表现为肿大,也可以表现为肥大。前者主要由炎症、过敏等因素刺激、粘膜肿胀、血管扩张、固有层下方水肿等所致,后者为粘膜在上述病变的基础上,当病情迸一步发展时,血管周围的纤维组织增生,严重时甚至引起鼻甲骨膜或骨质的增生,而致鼻甲肥大。无论鼻甲肿大或肥大,鼻粘膜纤毛功能均遭到损害;同时由于变大的鼻甲占据了鼻内有限的空间,影响通气,可产生相应的临床症状。治疗的主要目的在于改善鼻通气,辅助鼻粘膜生理功能的恢复。 下鼻甲的处理 2006-7-5 14:41:36 下鼻甲增生肥大,是引起鼻通气阻力增高的重要原因。引起下鼻甲肥大的原因有多种,如长期慢性鼻腔及鼻窦炎症刺激下鼻甲粘膜肥厚增生,甚至桑葚样变;变应性鼻炎及哮喘患者的下鼻甲水肿增生;下鼻甲在鼻中隔偏曲对侧代偿性肥大等。为提高鼻通气度,针对下鼻甲肥大的下鼻甲手术有多种术式,目前下鼻甲手术方式主要有下鼻甲电烧灼术、冷冻、激光炭化及下鼻甲部分切除术等。 Mabry(1988年)对下鼻甲肥大及治疗做了一系列研究。下鼻甲肥大且鼻阻经药物保守治疗无效,应考虑下鼻甲手术。对下鼻甲部分切除术须粘膜下切除增生的下鼻甲骨。提倡下鼻甲成型手术,即按照下鼻甲形态和改善通气设计的下鼻甲部分切除术。但随访5年后组织病理学仍提示下鼻甲纤维化、瘢痕化,粘膜中的粘液腺减少。Illum(1997年)曾报告对鼻中隔偏曲患者偏曲对侧下鼻甲代偿性增生在鼻中隔矫正手术时行下鼻甲前下部分切除术,并设对照组,即单纯鼻中隔矫正术。但术后声反射鼻腔测量表明两组鼻腔立体结构虽有明显区别,但或者通气满意度并未有显著变化,表明下鼻甲并不是唯一影响鼻通气阻力的因素。 越来越多的临床的实践表明单纯行下鼻甲切除术非但不能彻底解决鼻通气障碍的问,还会带来一系列不良后果。Oburra(1995年)介绍34例双侧下鼻甲切除手术,手术后最常见的并发症为粘连(15%),萎缩性鼻炎(15%),持续鼻塞(12%)及鼻异常感(9%)。Carrie(1996年)报告下鼻甲部分切除后长期随访发现下鼻甲增生明显,鼻阻症状逐渐加重, 4this year ... ...
