道路修复施工 目 录 第一章 工程概况„„„„„„„„„„„„„„2 第二章 主要施工技术方案„„„„„„„„„„2 第三章 安全生产保证措施 „„„„„„„„„ 21 第四章 文明施工保证措施 „„„„„„„„„13 第五章 质量保证措施„„„„„„„„„„„ 28 第一章、工程概况 本工程位于淮南市八公山区(镇)沈巷村紫金山庄内。 庄内道路修复，100?厚碎石垫层，200?厚C25混凝土现浇路面,下水道翻修，采用ø400和ø800钢筋砼涵管。 第二章、 主要施工技术方案 (一)、施工测量 本工程施工控制网为方格网，采用直角坐标法及极坐标法进行综合放线，根据甲方提供的红线图进行定位，建立独立的平面控制网，严格按工程测量规范要求，准确地测设出建筑物的轴线和标高，并设置引桩和水准控制网，对整个工程进行控制。 1、测量人员及测量设备的配置 根据本工程规模及结构特点，拟选配经验丰富的测量工程师1名，测量员2名，负责整个施工过程测量放线工作。 2、建立施工平面控制网 根据业主移交的测量控制网资料和施工坐标与测量控制网关系，来建立建筑物的轴线控制网。 采用“内控法”，以SET20?型全站仪测设一级精度田字形及扇开主控线，作为整个工程轴线控制的基准。 二级精度控制网:由于本工程场地较大，分区施工，一次整体难以做到，故除主控线外，各区均应设置控制点，并在工程场地外较稳固的地方布设3,5 个控制点，无论各区施工情况 采用内控、外控相结合方法既保证整个工程结构的整体性，又能保证独立性。 3、主轴线的定位与放线 在基础施工时，采用“内控法”，用SET20?型全站仪将一级精度控制网引测到基槽，依据这些控制线，辅以苏光J2经纬仪及50M钢卷尺逐次放出所需轴线，主控制线的标定，在施工前期采用50mm×100mm×1000mm木桩钉设，木桩钉设的控制轴线的使用期限不得超过两天，且每次使用前必须校核木桩有无移动。 在主体结构施工时，采用“内控法”与“外控法”相结合的方法，根据底板经复核的轴线控制点引测到?0.000m层楼板上，埋设控制铁件，用“内控法”以检定合格的SET20?型全站仪测试级精度田字形及扇形主控线，作为本工程轴线控制和基准。由于本工程分区施工，由主控线测设条一级精度的轴线控制网，在各控制轴线埋设激光控制点，并在上部楼层的相应位置留设100mm×100mm的预留孔，用于轴线上下传递，辅以苏光J2经纬仪及50m钢卷尺逐一放出所需轴线。 2 考虑到本工程单层面积大，且分区施工，一次整体控制难以做到，可采用建立的二级精度控制网，确保各区能顺利施工。各层施工轴线均从首层埋设精度控制网，确保各区能顺利施工。各层施工轴线均从首层埋设的激光对中点引测，用经纬仪将控制点垂直投测到所需的层面上，根据控制轴线逐次放出所需的轴线，为保证控制的引测精度，对控制轴线分二次进行整体监控，?0.000~而三层结构层一次，局部四~六层结构层一次，用全站仪对田字形主控线进行复测，从而保证主体结构轴线的精度。 4、高程控制 基础施工阶段根据水准控制网从水准控制点引到施工控制标高，与相邻水准点对照闭合，因场区较大，可在场区内布置6个高程起始点，与已知高程点构成闭合水准路线进行测量。一层结构施工时，将建筑物?0.000标高引测到一层结构内，以上各层的高程通过激光孔用水准仪和钢卷尺来引测到各层楼面，进行误差校正，即为楼面标高，并用水准仪抄平，弹出楼面标高?0.500M处水平线. (二)、土方与基础施工 本工程基础土方及施工场地多余的运出。土方开挖采用机械开挖、人工辅助的方法进行。先挖地下室、框架楼，在挖其他栋号基础。 1、测量定位 首先根据建立的施工测量平面控制网，根据基础平面布置在地面测设控制轴线，确定开挖路线、开挖方法。 2、土方开挖 、土方开挖前，应摸清地下管线等障碍物，并应根据施工方案的要求，将施工区域内的地上、地下障碍物清除和处理完毕。建筑物的位置和场地定位控制线(桩)、标准水平桩及基槽的灰线尺寸，必须经过检验合格，并办完预检手续。采用机械开挖，人工配合的方法进行。现场配置WY—100型反铲掘机4台、自卸汽车15辆，分三个区域同时进行土方基础开挖。 土方开挖时必须密切配合测量放线， 严格控制标高、几何尺寸，严防超挖、少挖现象，机械开挖时，坑壁、坑底、基坑边及地梁壁、底均应留安排工序，防止错挖或超挖，施工场地应根据需要安装照明设施，在危险地段应设置明显标志。放坡系数为1:0.6为防止超挖和保护边坡坡度的正确性，机械开挖至接近设计坑底标高或边坡边界，预留200mm厚土层，用人工清土和修坡。挖土时发现异常情况及时汇报，并进行探查处理。 、临时堆土及用作回填的土方，距基坑确定干燥土不小于3m，松软土不小于5 m，留置回填的土方根据土质透水性大小分类堆放，并堆土成方。余土及时运出。 3 、基础挖好后，对基础进行抄平、修正。 ?、基础开挖到设计深度，经核定符合设计要求后，再用人工清底，不得扰动基地。为防止坑底土质扰动，基坑挖好后尽量减少暴露时间，及时进行基础垫层施工。如不能立即进行下一道工序时，预留15cm~20cm厚覆盖土层，待基础施工时再挖去。 基础清底完毕后，及时组织建设单位、设计院、质监站、监理等部门验收，合格后立即进行砼垫层施工。 ?、开挖出来的土方应作及时清理。先做好土方平衡计算，征求建设单位意见，需要填部分尽量不外运，等按要求回填后，才把多余的土方运到建设单位指定的弃土点，以节约投资。多余的土方应一次运至弃土处，避免二次搬运。 ?、土方工程不宜在雨天进行。 (三)、基础施工 基坑开挖完毕后，现场验槽然后方能施工基础。 基础施工时应用龙门桩确定基础轴线、边线及标高。 箱筏基础采用组合钢模，其钢筋、混凝土等工序施工按相关技术规范和设计要求进行，确保基础施工质量。 条形基础模板安装 先在基槽底弹基础边线，再把侧板对准边线垂直竖立，用水平尺校正侧板顶面水平后，再用斜撑和平撑钉牢。如基础较长，应先安装基础两端的端模板，校正后，再在侧板上口拉通线，依照通线再安装侧板，为防止在浇筑混凝土时模板变形，保证基础宽度的准确，在侧板上口每隔一定距离钉上搭头木。 (四)、土方回填 采用人机械结合回填方案:按分层夯填标准，由人工摊平，打夯机夯实。 (1)填土分层进行，每层虚铺土厚度200mm—300mm。 (2)粘土或排水不良的沙土用于回填，通过击实试验测定其最优含水量与相应的最大密实度。土的含水量过大，采用翻松、晾干、风干、换土回填，如土料过干，预先雾状喷水湿润调整。 (3)在气候干燥时，采取速取速填，减少水分散失。填土尽量采用同类土堆筑，当采用不同土时，将透水性大的土层置于透水性小的土层之下，有规则的分层，不混杂。与墙体接触部分按设计要求进行。 (4)采用冲击式打夯机夯实，打夯前，初步平整，依次夯打，均匀分布，不留间隙。 (5)回填土分层检验。 4 如果利用基坑中挖出的不含有杂质的土进行回填，其粒径不大于50mm，含水量符合规定。 回填前，对基础墙体进行隐检，并且结构砼达到规定强度后进行。 施工前根据工程特点、土质、设计压实系统、施工条件等合理确定填料含水率控制范围，铺土厚度和夯实遍数。 每层施工完毕，进行环刀取样试验，压实系数不小于0.9后方进行下一层的施工。 (6)回填前做好标高标志的设置。 回填土分层铺摊和夯实，铺土厚度和夯实遍数根据土质、压实系数和机具性能而定，每层铺厚为200mm~250mm，采用蛙式打夯机夯实，每层夯打三遍，并且一夯压半夯。 基坑回填在相对两侧或四周同时进行，基础墙两侧回填标高不能相差太大。 基坑回填连续进行尽快完成，施工中防止地面水流入坑中，现场有防雨排水措施。 施工中注意保护施测桩线，防止碰撞移位。 三、主体施工 (一)、模板施工 1、模板安装施工工艺 柱模板支设顺序:检查坂平整度?侧模模板安装就位?安装纵横套管? 穿对拉螺栓?支设支撑?调整垂直度?紧固支撑。 梁模板支设顺序:搭设梁模脚手架?铺设梁底模板?安装梁侧模?安装梁测模坂钢管椤?复核梁模尺寸标高?与相邻柱的连接固定。 楼面模板支设顺序:搭设模板支撑架?铺设枋木(50×100木方间距300mm)?铺设组装模板?复核模板平整度及标高。 2、梁、板模板施工 (1)、梁板模板施工工艺;放线定脚手架立杆位置?搭设脚手架?梁底模?梁?侧模、板模?绑扎钢筋?封梁另一侧模、板模?绑扎梁板钢筋?设立快拆支撑及支撑加固。 (2)、梁板模 梁板模采用满堂钢管脚手架支撑。模板背楞木方间距约300。立杆间距约1.2m×1.2m。支撑架水平杆用3道，适当增加剪刀撑。梁高度均大于600，需加对拉螺杆，纵向间距500，横向间距视梁高度而定。当梁跨度大于4 m时，按跨度约1/400适当起拱。先主梁起拱，后次梁起拱。 5 (3)楼梯模板 现浇砼楼梯采用木模板，钢管脚手架支撑与主体施工同步进行，为上下提供方便。施工前根据实际层高放样，先装平台梁及基础模板，再装楼梯斜梁或楼梯底模板，然后安装楼外帮侧板，外帮侧板应先在其内侧弹出楼梯底板厚度线，套画出踏步侧面置线，钉好固定踏步侧板的挡木，在现场装钉侧板，并用对拉螺栓固定，对拉螺栓为每三个踏步设一个。楼梯高度要均匀一致，特别要注意粉刷厚度和饰面厚度，防止出现踏步高度不等。 4、模板施工要求 ?模板使用前要刷脱模剂，拆除后要清除表面砼及铁钉。 ?模板搭设按设计要求进行，扣件紧固，支架稳定。 ?模板拼缝密实，有缝隙处用白铁皮补缝。 ?墙、柱模支好后，检查位置是否正确，吊垂线检查垂直度是否符合要求，拉通线检查是否顺直。达不到要求进行整改。 ?梁板模支好后，用水准仪检查平整度情况，符合要求及标准才准下道施工。 ?在封模前，检查预埋件、预留洞是否遗漏。位置是否正确。杂物是否清除。 ?砼强度达到拆模强度要求才准拆模。悬挑梁、板必须待砼强度达到100%设计强度后，方可拆除底模。 ?在砼浇筑中如发现变形、松动现象及时修整加固。 9?梁板模配置等施工图出来后再作具体的施工配置编号图。 5、模板配置 柱模:配置一层 梁、板模板:采用快拆支撑体系，可在砼浇筑完3—5天便加拆除一般支撑，保留快拆头支撑部份，加快模板、钢管、扣件的周转使用，减小投入，提高周转架料的使用率。周转架料配备见周转架料计划。 6、模板工程质量标准 1) 模板施工时，必须派专人管理预埋件及预埋洞口，检查钢筋保护层、垫块，要制定加固措施，做到不漏，安装牢固，位置准确。 2) 拆模板时不得硬撬硬砸，以免损伤砼表面和拆下的模板。模板要有专人管理，及时清理除渣、校正刷油并分类堆放，避免混用，以利于周转使用和提高周转使用率。为防止漏浆，模板校正加固完毕后，应在墙模、柱模根部外侧抹10—20mm厚，宽50mm左右砂浆封堵空隙，梁跨大于4m时应按跨长的1?-3?起供。 3) 模板安装允许偏差表 6 7、模板工程安全技术措施 4) 支模过程中应遵守安全操作规程，模板不得空架浮搁。 5) 拆楼层外边模板时，应防高空附落。 6) 安装拆除外墙模板时，必须确认外脚手架符合安全要求。 7) 正在施工浇捣的楼板其下一层楼板的支顶不准拆除。 8) 安装二层或二层以上模板，应先搭设脚手架或安全网。 9) 大于6级风时，停止大模板吊装。 10) 悬挑部位的模板应等砼强度达到设计要求后方能拆除。 6、模板工程文明施工措施 1) 模板加工应集中在专门的木工房内进行，减小噪音，防止木屑满天飞扬。 2) 已加工的械板应分类编号堆放，铁钉、穿墙螺栓等派人统一管理，集中堆入，防止伤人。 3) 严禁在楼层上拉锯模板，防止木屑、废木块混入已绑扎好的钢筋中，而影响砼工程质量。 (二)、 钢筋工程 一)、钢筋采购、加工 1、 钢筋的进场:所有运至现场的钢筋，其品种、牌号、质量等级均应符合设计要求及招标单位质控材料一览表的要求，进厂时应有出厂质量证明或厂家的试验报告单，钢筋表面或每捆(盘)钢筋均应有标志，且与报告单中批号吻合，否则不得进场。 2、 钢筋的验收;成批钢筋进场时，应按批进行检查和验收，并填写进场检验记录。每批由同牌号、同炉罐号、同规格、同交货状态的钢筋组成，重量不大于60吨。 3、 钢筋的检查 1) 钢筋的外观检查包括:直径、标牌、外形、长度等项目，对牌号不符的钢筋拒绝使用。 2) 力学性能试验;从每批钢筋中任选两根钢筋，每根取两个试样分别进行拉伸试验和冷弯试验。 3) 试样的取样部位为;从材料端部沿钢筋扎制方向切取，如该部扎制头未切去者，需离端部500mm处切取试样,切取试样可采用烧断法、剪切法。 4、 钢筋的加工与堆放 1) 钢筋的堆放:本工程所有钢筋均在施工现场加工成型。存放钢筋的地方铺设碎卵石硬化并设排水坡，四周挖设排水沟。钢筋堆放采用砌砖台架空处理，将钢筋原料、半成品堆放 7 于砌砖上，下雨时用彩条布覆盖，以防钢筋锈蚀和污染。钢筋距地面高度大于300mm，应注意“先进先用、后进后用”的原则。 2) 钢筋下料单的编写:设专人严格按规范、设计图纸、变更洽商和图集进行钢筋翻样，编写钢筋下料单。下料单经过技术负责人审查后方可进持加工。 3) 现场管理:建立严格的钢筋生产、安全#管理制度#，并制定节约措施，降低材料损耗。 4) 盘条钢筋的调直采用卷扬机拉直或用钢筋调直机。粗钢筋的调直主要采用锤直和扳直的方法进行。钢筋的下料采用钢筋切断机，钢筋的弯曲采用钢筋弯曲机。下料前要熟悉图纸，依据图纸审核料单，下料过程中，注意设计和规范的各项要求，钢筋的弯折长度、弯折角度、搭接长度、平直长度以及高度等都需注意，发现问及时与技术人员联系，防止下料中尺寸出现偏差。 5) 钢筋加工的一般要求:?级钢筋的未端需作180º弯钩，其圆弧弯曲直径不应小于钢筋直径的2.5倍，平直部分长度不宜小于钢筋直径的3倍。?级钢筋的未端按需要做90或135º 弯折，弯曲直径不宜小于钢筋直径的4倍，平直部分长度按图纸设计要求。箍筋的弯曲直径应大于受力钢筋的直径，且不小于箍筋直径的2.5倍，弯钩平直部分长度不应小地箍筋直径的10倍。 6) 钢筋的标识: 成型的钢筋半成品在堆放时,同一部位或同一构件的钢筋要堆放在一起,并用塑料布料牌进行标识,标识上注明构件名称、部位、钢筋型号、外观形状、直径、根数、检验状态等。 7)制作的半成品钢筋，应在制作场内进行预检，合格要求后方可用于现场，并做好记录。 8)钢筋加工的允许偏差 项 目 允 许 偏 差 受力钢筋顺长度方向全长的净尺寸 ?10 弯起钢筋的弯折位置 ?20 (三)、钢筋绑扎施工 1、 柱钢筋绑扎 工艺流程: 焊接竖向受 画箍筋间距 绑 箍 套柱箍 箍 8 1) 绑扎前，首先检查纵向钢筋位置是否正确，如有偏位按安1:6的坡度进行调整。 2) 对竖向钢筋的接头进行外观检查及力学试验，合乎要求方可绑扎箍筋。 3) 绑扎箍筋时，采用皮数杆确定箍筋间距，然后将已套好的箍筋向上移动，由上向下绑扎。 4) 柱箍筋上下应连续，在梁柱节点部位应根据该区段的间距进行绑扎，严禁因操作难度大而漏扎。 5) 箍筋面应与主筋垂直绑扎，箍筋转角与主筋交点均需绑扎，并且要保持箍筋的弯直部分要在柱上四角通转。 6) 柱箍筋端头的平直部分与砼面保持450º夹角，平直长度不小于10d绑扎时，要特别注意柱子出现扭位现象，如发现可将部分箍拆除重绑。 2(梁、板筋绑扎 1) 工艺流程为:绑扎梁钢筋?清理模板?模板上画线?绑板下铁?水电配管?绑板上铁。 2) 梁钢筋绑扎应注意梁受力主筋的相对位置，以及与柱纵向受力主筋之间的关系，保证梁及柱主筋位置，其余箍筋相应缩小。 3) 梁钢筋绑扎时，应同时将保护层、一、二排筋间间距调整好，控制好加密箍筋及吊间距位置。支模时应注意成品保护。 4) 板绑扎前应清理掉板模上的锯未、碎木头、电线、管头等杂物，先摆放受力主筋，后摆放分布筋。 5) 单向板除外围两根筋相交点全部绑扎外，其余各点可交错绑扎，双向板相交点必须全部绑扎。 6) 双层钢筋网片之间根据板厚不同用ф12制作“A”型马凳铁，间距600mm，呈梅花状布置，保证上铁位置准确，如下图;下铁保护层采用15mm厚砂浆垫块控制。 7) 楼板钢筋绑扎及浇筑砼时，应做好成品保护工作;铺垫木跳板，操作人员在板上作业。 3(楼梯钢筋绑扎 1)工艺流程:弹钢筋控制线?绑主筋?绑分布筋?绑踏步筋。 2)先绑扎主筋后绑扎分布筋。每个交点均应绑扎，注意梯梁筋绑扎完后再绑板筋，板筋锚固到梁内。 3)底板筋绑完，待踏步模板支好后，垫好负弯矩筋支凳并绑扎固定。 9 4(钢筋绑扎质量要求 1) 钢筋进场后，检查其规格、形式、尺寸、数量以及外观质量是否符合要求。 2) 绑扎过程中，检查使用位置是否正确，间距是否准确，锚固长度是否满足要求，接头位置是否符合设计和规范规定。 3) 应检查保护层厚度是否符合要求、检查钢筋绑扎是否有松扣、漏扣现象及绑丝朝向是否正确;检查钢筋是否有油渍、漆污和铁锈(片状);检查钢筋焊接或机械连接质量;如有问题应及时整改。 4) 钢筋工程属隐蔽工程，在浇筑砼前应对钢筋及预埋件进行验收，并作好隐蔽工程检查记录，同时进行各专业联检手续应齐全。 (三)、 砼工程 一)、砼试配与选料 1、 砼试配与选料:严格按设计和规范要求控制原材料质量，结合招标单位质控材料一览表，选用级配良好，各项指标符合要求的砂石材料，水泥以选用同品种、同标号、同一生产厂家为好，各种外加剂也应选同品种，同一生产厂家的，一次浇筑量应备足，所选用的原材料必须检验合格后才能使用。 2、 中标进场后，立即组织对原材料的选择，积极实行见证取样、试验。砼配合比按设计按规范要求，并参考以往的施工级配，考虑到可能遇到的气候等外部条件的不利影响，优化配合比设计。配合比一经确认后，即应积极按要求备料，作好施工前期的准备工作。 二)、砼的供应 1?、搅拌方式:本工程全部采用商品砼。 2?、砼输送方式:大量砼有商品砼车送到各栋号，垂直运输采用塔吊和井架。 (三)、砼施工工艺 (1) 、工艺流程: 配合比 原材料准备 混凝土搅拌混凝土运输混凝土布料 外加剂 混凝土浇筑、振动 10 (2)、浇筑大体积基础，应分块分层进行浇筑，每层高约900，每次浇筑宽1.5m左右，以一定的坡度循序推进，每层保护盖住下层混凝土不超过初凝;为防止混凝土自流，使表面不平，采用支设模板隔挡，每段浇筑完后，立即拆除支挡模板。 (3)、浇筑混凝土落灰高度不宜超过2m，如超过2m，应先铺一层100厚的原混凝土配合比无石子的砂浆。 (4)、梁、板、柱砼浇捣 先清理工作面上各类垃圾，梁墙接头部位留出清扫口，冲完毕后，加以封闭，清理重点在施工缝及梁底，注意梁底与梁侧面振捣情况，振动器不要直接触及钢筋和预埋件，楼板砼虚铺厚度略大于板厚，用平板振动器或振动棒振实，用铁扦尺检查砼板厚度，振捣完后用长的木抹子抹平，在砼强 2 度未达到1.2N/mm时不得在楼面安装模板及支架. (5)、砼养护 砼浇筑完毕后根据现场实际情况开始养护。梁板结构采用浇水养护。砼养护时间不小于7天。 四)、砼结构防裂缝处理 1、 危害 砼浇筑中的质量问题，直接影响砼结构受力性能和砼强度，影响砼表面的质量标准。 2、 原因分析 砼离析:坍落度过大或过小，和易性差; 缝隙夹渣:砼施工缝留置和处理不符合要求，缝隙夹渣; 冷缝:出现施工冷缝; 存在垃圾:未及时清除薄膜和松动的石子、杂物、垃圾等; 级配不好:砼配合比计量不准，砂石级配不好; 下料不符:砼浇筑高度过大，一次下料过多，分层不清，振捣不实; 骨架错位:钢筋骨架加工绑扎不准，定位措施不力; 无排气孔:较大的预留洞，窗洞的下口模板无排气口，砼浇筑有气泡，形成孔洞; 模板未湿润:模板在浇筑砼前未提前湿润，浇筑后木模膨胀，砼角被拉裂; 支撑不够:模板未经设计计算，刚度不够，支撑或拉结不足，造成墙、梁断面尺寸鼓出，接头歪斜等。 3、 防治措施 11 配比准确:砼配合比由有资格的试验室设计，所用材料必须检验合格，砼搅拌时必须严格下料，计量准确; 缓凝:商品砼应据运距、交通条件、季节，采取加缓凝剂等措施，严禁二次加水; 支撑计算:模板应经设计计算，保证结构尺寸和支撑牢固; 设置气孔:预留洞和门窗洞口模板，四角应设斜撑，较大洞口底模要设排气孔; 接浆:为防止柱、墙烂根，对模板下口砼表面找平，抹50?80mm水泥砂浆找平层; 冲洗根部:在砼墙、柱、板已硬化的砼表面或施工缝处，对薄膜和松动的石子、垃圾应清除，冲洗干净; 湿润模板:砼浇筑前，对模板提前湿润。砼自由倾落高度不得超过2000mm; 振捣:砼应分层浇注，连接振捣; 下料:预留洞、门窗洞口浇筑砼时，砼从洞口两侧同时下料，高度大体一致，从两侧同时振捣，防止洞口变形。 模板拆除:拆除砼侧面非承重模板时，砼强度应达到1.2Mpa以上才能进行.拆模时不能用力过猛过急,不能生撬硬砸. 4、 砼表面缺陷的修整、处理 表面缺陷:面积较小且数量不多的蜂窝、麻面、磨石的砼表面，先用钢丝刷，将基层刷净，用水冲洗，再用1:2?1:25水泥砂浆抹平; 深层缺陷:面积较大的砼蜂窝、麻面、小孔洞，应按其全部深度凿去薄弱的砼层和突出的石子，用钢丝刷净，压力水冲洗，用比原砼强度高一级的细石砼堵塞、填实; 质量问题:对影响砼结构较大的缺陷，如大面积孔洞、梁板处施工缝较严重缝隙夹层，须会同设计、业主、质量监督等有关单位，共同研究，制定处理方案。 五)、砼施工注意事项 1、 按要求在现场对砼见证取样、送检。 2、 梁、板交界处二次振捣，保证砼结构整体性，板面一次性刮平二次收光，及时养护。 3、 振捣砼时，严格按规范要求进行振捣，禁止现场过振、漏振。 12 4、 在雨天时，不宜露天浇筑，如无法避免，则应对已浇捣的砼表面用编织布予以覆盖，雨后清理面层，并按施工缝的要求进行处理后再进行砼的继续浇捣。 六)、砼工程质量标准 1、 砼所用水泥、水、骨料、外加剂等必须符合施工规范及有关规定，使用前要检查出厂合格证或者检验报告是否符合质量要求。 2、 砼配合比、养护和施工缝处理必须符合施工规范规定，并作好施工日志。 3、 砼应振捣密实，无蜂窝、孔洞、露筋等现象。 4、 砼强度评定按国家GB50204-2001第四章执行。 5、 砼质量评定按国家标准GBJ301-88第五章执行。 七)、主要安全技术措施 1、 泵车应放置平稳，轮胎处垫三角木，以防滑动。 2、 楼面上的孔洞应予以遮盖或有其它保护措施。 3、 夜间作业应有足够照明设备。 (八)、文明施工措施 1、 进出车辆必须进行冲洗，防止污染路面，保持现场排水沟的通畅，防止污水回溢。 2、 砼浇完后，应及时清理剩余的砼，防止砼结块而影响现场通畅。 (四)、 砌筑工程 本工程砌体材料采用煤矸石空心砖 一)、原材料准备 1、 混凝土砌块和煤矸石砖的品种、强度等级必须符合设计要求，规格一致容重符合设计要求，有出厂合格证明及试验单，并说明制造日期和强度等级。 2、 水泥品种与标号应符合设计及规范要求;应有出厂合格证明和试验报告方可使用，不同品种的水泥不得混合使用。 3、 砂宜用中细砂，含泥量不应超过5%。 4、 石灰膏熟化时间不少于7d，严禁使用脱水硬化的石灰膏。 5、 施工前，应弹好建筑物的主要轴线和砌体的砌筑控制轴线边线及标高、门窗洞位置等，须复核无误后方可施工。 6、 砌块砌筑前一天，应将预砌的煤矸石砖砌块与原结构相接处浇水湿润，确保砌体粘结。 7、 砌筑前砌块需提出前一天进行浇水，使其充分湿润，保证其粘结质量。 二)、砌筑工艺 13 砂浆的拌制:砂浆的搅拌应严格执行砂浆配合比，砂浆必须采用机械搅拌，拌和时间不得少于1. 5min; 排砖撂底:根据墙体施工平面放线和设计图纸上的门、窗位置大小，层高、砌块错缝、搭接的构造要求和砂缝大小，对每片墙进行预排，以借助实砌使用。 砌筑:砌筑时应采用满铺满坐的砌法，然后将砌块按预排图就位，待就位后，即检查其垂直、平整度，并加以较正，镶砖补缺工作与安装坐砌紧配合进行，竖向灰缝可用加浆法填塞饱满。 砌块与柱相接:砌块与墙、柱相接位置，应按设计要求预留拉结筋，铺将时将钢筋理直铺平。 砌块与门、窗联接:将预制好的木砖或铁件按洞高2m内每边埋三块、2m外每边埋四块，用作固定门框用，门窗洞上方过梁支座范围内应用煤矸石砖砌自由式不少于四皮的实心砖墙，防止应力集中出现裂纹。 砌块与梁、板底的联接:在墙顶与梁底(或板底)间。用粘结砂浆斜砌一层，黏土烧结砖上下顶紧、灌浆堵实。 三)、砌筑要点 1、 砌块的品种、强度等级、容量等均必须符合设计要求; 2、 砂浆品种符合设计要求、强度符合规范要求; 3、 砌体砂浆必须密实饱满，砌体无通缝; 4、 拉结筋数量、长度、间距均要会合设计及施工规范规定; 5、 构造柱留置位置正确，马牙槎符合规范要求; 6、 砌筑时，对设计的洞口、管道、预埋件等，应同时进行预留或预埋，不得事后凿打。 四)、砌筑安全技术措施 1、 搅拌机的进料口上应装铁栅栏保护，传动皮带和齿轮必须装防护罩; 2、 搅拌前应检查搅拌页有无松动或磨刮自身现象，出料机械是否灵活，机械运转是否正常; 3、 搅拌机拌叶达到正常运转后，方可投料，转动时，不准用手或其他物体去刮拌和灰浆或材料。 4、 出料时必须使用摇手柄，不准用手转拌和筒，如遇故障或停电，应拉闸将拌料清除。 5、 砌块应提前湿润，不应到操作地点时才进行，以免造成场地湿滑。 6、 运输砂浆或砌块时不得高速跑步，保持车距在2m以上;如遇坡，应10m以上，禁止并行或超车。 7、 塔吊用作垂直运输时，其载重量不得超过吊运荷载能力。 14 8、 禁止抛砖运送，人工传递时，应稳递稳接，不能过于集中。 9、 活动脚手架应搭设牢固，底部应垫平，脚手板不得少于两块，端头伸出支承杆约20cm，脚手板每块上的操作人员不应超过两人，脚手板上宜一块板堆砌块，一块站人。 四、下水沟施工 (1)、老路面拆除 1、拆除前在路中弹好墨线，用切割机顺线切割，在用捣机破碎。 管道沟开挖前，先摸清地下管线，以防损坏管线要绘制基坑开挖图;施工时，按图放样开挖，在底板周边必须留足工作面，周边开挖排水沟; 2、管道沟开挖完后立即组织人员进行沟槽清理，干铺碎石垫层，浇筑C15砼。 (2)、基坑安全防护 a、沿坑底周边设一道防护栏杆，距坑边0.5M，栏杆用红白漆间隔涂刷显目的花杆或架拉一道色带。防护围栏要牢固，要能承受不小于1千牛的外力，以防有人不注意掉入坑内摔伤。 b、设置围栏高度为1.2M，栏杆应设置两道水平杆，下道0.6M高，上道1.2 M高，栏杆柱间隔不大于2M，上悬挂安全牌及夜间安全照明灯。 (3)、 施工排水 排水沟应布置在底板四周工作面以外，四周连通，沟坡要稳定，决不允许在基坑中间挖排水沟。沟底应比基坑底板底面超深30—50CM。并设置排水井与排水沟相连，排水井深度要低于排水沟50CM以上。施工期间，机泵排水要有专人清理疏通龙沟，确保排水畅通，达到底板无水。地平面可采用挖临时排水沟或筑土堤等措施，阻止场外水流入施工场地。 (4)、 回填土 ?、对填方基底和隐藏工程要先进行检查和中间验收并作好记录。 ?、回填土需做到:填土前必须清除碎砖石等杂物，并排除积水。从低处开始，平坯上土，每层上土厚度不超过20—30CM，夯打三遍以上，回填土要逐坯夯实，靠墙边用木棒、石槌仔细夯实，回填土密度应达设计要求。 (二)、道路基础处理 (1)、采用碎石填平路基。 、在路基上进行中线测量，敷设适当桩距的中心桩，并在路面边缘处设置导线桩。 、清除路面范围内的杂物，原路砼面有局部路基坑洼找平，温软、弹簧、应挖开晾晒，在土壤最佳含水量时回填压实。 15 (2)碎石找平层 、用碎石按松铺计算碎石所需数量,运至路基上,分段堆放. 、将准备好的碎石按松铺厚度一次铺足, 松铺系数为1.2-1.3左右,用18T压路机压实,压实厚度符合设计要求,压实密度不得低于93% 。 (3)、模板安拆 1、模板采用钢模板，用200mm×40?槽钢制成，长4m，高度与混凝土板厚度一致。 2、模板高度的允许误差为+2mm，企口舌部或凹槽的长度允许误差，钢模板+1mm。 3、立模的平面位置与高程应符合设计要求，并应支立准确、稳固。接头紧密平整不得有离缝、前后错茬和高低不平的现象。模内侧要清洁，与混凝土接触的表面涂隔离剂。 4、支模时用φ18×50cm铁撅打入基层固定，外侧铁撅间距不大于60cm，内侧铁撅间距不大于1.2m，外侧铁撅顶不高出模板，曲线处内外侧铁撅要加密，使弯曲圆滑，支模后稳定坚固，铁撅与模板要靠紧，如个别有空隙处要加木块，并钉在模板上，避免振动时木块脱落模板变形，两块木模板接头处外侧要加挡板，长度不小于80cm，以避免错口。 5、模板拆除应符合下列规定 拆除时间应根据气温和混凝土强度增长情况确定，采用普通水泥时，一般允许拆模时间见下表。 (1)允许拆模时间，自混凝土成型后至开始拆模时计算。 (2)使用矿渣水泥时。允许拆模时间宜延长50%—100%。 拆模应仔细，不得损坏混凝土板的边、角、尽量保持模板完好。 (三)、砼路面施工 1、砼强度等级: C25 2、砼试配与选料 (1)、砼试配与选料:严格按设计和规范要求控制原材料质量，结合招标单位质控材料一览表，选用级配良好，各项指标符合要求的砂石材料，水泥以选用同品种、同标号、同一生产厂家为好，各种外加剂也应选同品种，同一生产厂家的，一次浇筑量应备足，所选用的原材料必须检验合格后才能使用。 3、 混凝土采用商品砼。 4、 混凝土拌和物的摊铺和振实 16 (1)、混凝土拌和物摊铺前，应对模板的间隔、高度、润滑、支撑稳定情况和基层的平整。润湿情况以及钢筋的位置和传力杆装置等进行全面检查。 (2)、在铺前纵向企口缝及路面与收水井、检查井的接缝处，应全面均匀涂抹刷沥青油，纵缝混凝土刷油应在表面稍干时进行，(此时混凝土板应暂停养护)。 (3)、混凝土板采用一次摊铺，摊铺厚度应考虑振实预留高度。 (4)、混凝土拌和物的振捣; 混凝土板在靠边、角处应先用插入式振捣器顺序前进，其移动间距不应大于其作用半径的1.5倍，至模板距应不大于其作用半径的0.5倍，并应避免碰撞模板与钢筋。再用功率不大于2.2kWh平板振捣器全面振捣，由板边向板中慢速前移，达板中后即快速移到板边，再慢速向板中移动，按顺序前进。每次需重叠1/3或10—20cm，然后纵向一行一行地由边向路中央移动振实。 振捣器在每一位置振捣的持续时间，应以拌和物停止下沉不再冒气泡并出水泥砂浆为准，不宜过振。用平板式振捣器振捣时。不宜少于15s;水灰比小于0.45时。不宜少于30s;用插入式振捣器时，不宜少于20s。 振捣时应辅以人工找平，并随时检查模板，如有松动、变形、下沉应及时纠正。 在振实和大致振平后，即用振动夯松(振动梁)往返振刮2—3遍，使表面泛起整平赶出气泡，前进速度以1.2—1.5m/min为宜。跟锨人员见有不平之处应及时挖填补齐，填补时要用碎石较细的混凝土拌和料，严禁用纯砂浆填补找平。振实夯板操作时，应平行移动，不允许中途停留，夯板底不得倾斜。振动夯板(振动梁)刮平后，再用滚浆筒进一步回拉提浆整平，设有路拱时，应使用路拱成形板整平。整平时必需经常检查振动梁底面的平整。 混凝土板的摊铺振实工作应连续进行， 不得中途间断，若遇特殊原因被迫临时停工，中断施工的一块混凝土板，应用湿布覆盖，在初凝时间内恢复施工时，应将此处混凝土耙松补浆后再继续浇筑。 若停工超过混凝土初凝时间，应作施工缝处理施工缝应庙在在缩缝处，若无法设在伸缩缝处，其位置应设在板在正中部位。 超过初凝时间的混凝土混合料，严禁使用严禁分散铺于混凝土板底层，已经摊铺的必需清除。 5、 混凝土板抹面及拉毛 (1)、必需严格掌握混凝土收水时间，及时做好收水抹面工作，以增强抗磨能力，防止产生网状细裂缝，收水抹面一般为四遍，第一遍必须在整乎完毕后的15min(根据气候掌握)后进行，其目的主要是 17 驱除泌水和压下石子。第二、三遍继续驱除泌水外，并应进行整平抹面工作。当混凝土处于初凝终止状态，表面尚呈湿润时，应趁时机进行第四遍抹面。把混凝土表面砂浆进一步挤压紧密。 (2)、在接缝处尤应注意整平。相邻板面高差不应大于质量标准。也应对板面随时用3m直尺检查平整度。及时作必要修正。 (3)、抹面工作必须在工作桥上进行。严禁站在混凝土板上操作。 (4)、人工抹面中禁止采用洒水、撒干水泥、铺浆等方法找平，一切修铺工作应在抹面前作好。 (5)、拉毛: 抹面后沿横坡方向采用机具压纹，压纹深度一般为1—3mm，其上口稍宽于下口。 6、接缝施工 (1)、膨胀缝 胀缝宜的设置应按设计要求确定。 胀缝的填缝板设置于混凝土板胀缝的下部，其高度为混凝土板厚的2/3，上部1/3处预埋木板制成的临时嵌缝条，在面层收水抹面时轻轻提起取出，填缝板和临时嵌缝板的接触面必需紧密，以防浇筑混凝土时砂浆流入影响胀缝作用。 胀缝应与路面中心线垂直，缝壁必须垂直;缝隙宽度必须一致，缝中不得连浆。缝的上口边缘应用边角抹子抹成圆角，缝隙上部应浇筑填缝料。 胀缝传力杆的活动端可设在缝的一边或交错布置。固定后的传力杆必需平行于板面及路面中心线，其误差不得大于5mm。传力杆的固定可采用顶头木模固定或支架安装的方法，应符合下列规定: 支架固定传力杆安装方法，宜用于混凝土板连续浇筑时设置的胀缝，传力杆长度的一半应穿过胀缝板和端头挡板，并应用钢筋支架固定就位，也可用定位板固定。浇筑前应先检查传力杆位置，再在胀缝两侧摊铺混凝土和物至板面，振捣密实后抽出端头挡板，空隙部分填铺混凝土拌和物，并用插入式振捣器振实。 (2)、伸缩缝 缩缝的施工办法采用切缝法。 切缝法施工，宜在混凝土强度达到设计强度25%—30%时即应进行锯缝，切不可延误锯缝时间，以免板自行断裂。施工时可每隔20—30mm先锯一条缩缝，再补锯其中缩缝。锯缝深度为40mm，缝宽为5mm。切缝用水冷却时，应防止切缝水渗入基层和土层。 (3)、施工缝 施工缝的位置宜与胀缝与缩缝设计位置吻合。 施工缝应与路面中心线垂直。 18 (4)、纵缝施工 纵缝采用平缝施工工艺，对已浇混凝土板的缝壁应涂刷沥青，并应避免涂在拉杆上，浇筑邻板时，缝的上部应压成规定浓度和缝槽。 纵缝的拉杆应相邻两板各占一半长度，中间设置拉杆的纵缝模板，应预先根据拉杆设计位置放样打眼。 7、 混凝土板养护 混凝土板的养护采用塑料薄膜养护，塑料薄膜养护应符合下列规定: (1)、塑料薄膜溶液的配合比应由试验确定。薄膜溶剂一般具有易易燃或有毒等特性，应做到贮运和安全工作。 (2)、塑料薄膜养护采用喷洒法，当混凝土表面不见浮水和用手指压无痕迹时，应立即进行喷洒。 (3)、喷洒厚度宜以能形成薄膜为度，用量控制在0.2—0.5kg/m2左右，不同养护剂应采用不同用量。 (4)、在高温、干燥、刮风时，在喷膜前后应用遮阴棚或其它措施加以遮盖。 (5)、养护期间应保护塑料薄膜的完整，当破塑时应立即修补。薄膜喷洒后3d内应禁止行人通行。养护期间和填缝前禁止一切车辆 行驶。 (6)、如成膜前遇雨淋，雨后应补喷。拆除边模后边部亦补喷塑料 薄膜溶液。 8、 填缝 (1)、混凝土板养护期满后，缝槽应及时填缝。在填缝前必须保持缝内清洁，防止砂石等杂物掉入缝内，常用的填缝材料可按本规范附录选用。 (2)、填缝采用灌入式填缝施工。 灌注填缝材料必须在缝槽干燥状态下进行，填缝材料应与混凝土壁粘附近紧密不渗水。 填缝材料的灌注高度夏天宜与板面平，冬天宜稍低于板面(约2—3mm)。 热灌填缝材料加热时应不断搅拌均匀，直至规定温度。当气温低时，应用喷灯加热缝壁。施工完毕应仔细检查填缝材料与缝壁粘结情况，在有脱开处，应用喷灯小火烘烤使其粘结紧密。 9、 质量保证 (1)为帮保证混凝土施工质量达到施工详图和本条款规定的要求，我们将对混凝土的原材料，配合比、施工过程各主要环节及硬化的混凝土质量进行严格的控制和检查。所进行的检查试验，按本条款规定所必须的混凝土质量控制和检查工作。 19 (2)配制混凝土所用水泥，应按本规范第6.4条规定进行试验检查，检查项目为:水泥标号、凝结时间、体积安定性。必要时，应增加稠度、细度、比重和水化热试验。 (3)掺用的煤灰等混合材料，每使用100-200t，应抽样检查其细度，需水量比、烧矢量、掺混合材的标准水泥砂浆强度等。对于细度和需水量比，每天至少检查一次，连续10个样品中，其个别样品的细度与平均值相差应不大于10%-15%。 (4)配制混凝土所使用的外加剂应有出厂合格证。当储存时间长，对其重量有怀疑时，必须按现行国家标准进行质量鉴定。严禁使用变质的不合格的外加剂。现场掺用的减水剂浓缩物，5T为一取样单位，加气剂以200Kg为一取样单位。对配制的外加剂溶液浓度，每班至少检查一次。 (5)拌和及养护混凝土所用的水应每季度检查一次。在水源改变或对水质有怀疑时，应随时进行检查。 (6)砂、石骨料的检查 1?、在筛分场应检查各级骨料的超径，含泥量，砂的细度模数等，每班检查一次。 2?、在拌和场应检查砂，碎(卵)石的含水量，砂的细度模数，经及骨料的含泥量、超逊径。砂、石的含水率变化每班应检查二次，宜分别控制在0.2时，则需调整混凝土的配合比。骨料的超径、含泥量每班应检查一次。此外，每季度应对骨料进行一次全分析检查。 (7)在拌和厂应检查各种原材料的配合量，每班至少三次。衡量应随时校正。为保证混凝土拌和均匀，应经常检查拌和时间是否符合规定，每班至少抽查二次。同时对一盘混凝土按出料先后各取一个试样， 3每个试样不少于30kg，测定砂浆容量，其差值应不大于30kg/m。用洗分析法测定粗骨料在混凝土中所占的百分比，其差值不应大于10%。 (8)混凝土坍落度的检查，每班在机口应进行四次，在仓面应进行两次。此外，在取样成型时，应同时测定坍落度。实测混凝土的坍落度与要求坍落度的偏差，应符合下表的规定。 (9)掺外加剂的混凝土，每班至少应检查二次含气量，其变化范围应控制在0.5%以内。 (10)现场混凝土的质量检验,以标准试件的混凝土抗压强度为主。必要时，尚需作抗拉，抗冻、抗渗等试验。确定结构构件的拆摸，吊运及施工期间临时负荷时的混凝土强度，应采用与借结构构件同条件养护的标准尺寸试件的混凝土强度。 (11)用于检查混凝土质量的试件，应在混凝土的浇筑地点随机取样制作，不得任意挑选。试件的留置，应符合规定:每一工作至少成型试件一组。 20 第三章、安全生产管理措施 安全工作是搞好生产的重要因素，关系到国家、企业和职工的切身利益，因此在施工过程中必需认真贯彻“安全第一、预防为主、综合治理”的方针，广泛应用安全系数工程和事故分析方法，严格控制和防止各类伤亡事故，具体措施如下: (一)、 安全生产管理 加强领导，健全组织，项目经理部成立安全领导小组，制定严格的安全措施，定期分析形势，研究解决施工中存在的问题。建立健全各级安全岗位责任制，责任落实到人。充分发挥各级专职安检人员的监督作用，及时发现和排除安全隐患。 1、 安全生产措施 1?、建立定期的安全检查与奖罚制度。我单位对工地安全生产实行月检，项目部日检，对检查出的安全隐患，实行定人，定时间、定措施，落实改革，对于违章人员按规定实行经济和行政处罚。 2、 各项经济承包有明确的安全指标和包括奖惩办法在内的保证措施。 3、 安全教育要经常化、制度化，提高全体员工的安全意识，切实树立安全第一的思想，建立完善的安全工作保证体系。对特种作业人员必须经培训合格后持证上岗;对新职工及工必须进行项目部、工区和班组三级安全教育和定期培训;通过安全竞赛、现场安全标语、图片等宣传形式，增强全员安全和产的自觉性，时时处处注意安全，把安全生产工作落实到实处。 4、 严格安全监督，建立和完善定期安全检查制度。各级安全领导小组要定期组织检查，各级安全监督人员要经常检查，真正把事故消灭在萌芽状态。 5、 施工现场按规定悬挂“七牌二图”与安全标牌。 6、 加强安全防护，设置安全防护标志，施工作业设立安全标杆、安全网。还应有保卫人员24小时值班。作业人员严禁酒后上岗，严格遵守操作规程，对违章作业又不听阻都将从严惩处，绝不姑息迁就，创造一个重视安全、处处遵章、文明施工的环境。 7、 抓好现场管理，坚持文明施工，保障人身、机械和器材的安全。在行人道路处开挖基坑及施工危险地段要设置安全警示牌，以防意外事故发生。 8、 认真做到防洪、防火、防雷工作，驻地和库房远离洪区，重点设备，重点防护对易燃、易爆、有毒器材按有关规定妥善保管，登记造册放置，责任落实到人，彻底消除不安全因素。 9、 定期或根据施工需要发放和检查施工所用的各种安全机具设备和劳动保护用品。 21 (二)、 施工安全用电和电气防火措施 1、安全用电措施 (1)、接地与接零 在施工现场专用的中性点直接接地的低压电力线中，必须采取TN-S接零保护系统(即三相五线制)。 A、保护零线应由工作接地线或配电室的零线或第一级漏电保护器电源侧的零线引出; B、保护零线应与工作零线分开单独敷设，不作它用，保护零线PE必须采用绿/黄双色线; C、保护零线必须在配电室(或总配电箱)配电线路中间和末端至少三处作重复接地，重复接地线应与保护零线相连接; D、保护零线的截面应不小于工作零线的截面，同时必须满足机械强度的要求，其中，架空敷设间距不大于12米时，采用绝缘铜线截面不小于10MM2，采用绝缘铝线截面不小于16MM2;与电气设备相连接的保护零线为截面不小于2.5mm2的绝缘多股铜线; E、电气设备的正常情况下不带电的金属外壳、框架、部件、管道、轨道、金属操作台以及靠近带电部分的金属围栏、金属门等均应用保护接零; F、供电电力变压器中性点的直接工作接地电阻值应不小于4欧姆，保护零线重复接地电阻值应不小于10欧姆。不得一部分设备作保护接零，另一部分作保护接地。 (2)、配置漏电保护器 A、施工现场的配电箱(配电室)和开关箱至少配置两级漏电保护器; B、漏电保护器应选用电流动作型，一般场合漏电保护器的额定漏电动作电流应不大于30MA，额定漏电动作时间应不大于0.1s;潮湿和有腐蚀介质场所的漏电保护器，其额定漏电动作电流和额定漏电动作时间乘积的极限值为(大于)30MA.S; C、开关箱内漏电保护器的选用应与动力设备的容量大小、相数等实际情况相适应、相配合，如三相电动机则应选用参数匹配的三相三线的漏电保护器;照明用电必须与动力用电分开，照明应选用单相二线的漏电保护器。 (3)、开关箱按三级设置，即总配电箱 分配电箱 开关箱，开关箱距离机具不能超过三米，开关箱实行一机一闸一漏电保护。 (4)、外电保护 A、在建工程的外侧边缘与外电架空线路的边线之间和最小安全操作距离:电压1KV以下为4米;电压1KV-10KV之间为6米; B、施工现场的机动车道与外电架空线路交叉时的最小垂直距离:电压1KV以下为6米，电压1KV-10KV之间为7米; 22 (5)、配电系统 A、所有的电线架设都必须使用电杆、绝缘子、横担等，按规范要求架设; B、开关电器及电气装置必须完好无损; C、开关电器及电气装置必须装设端正、牢固，不得拖地放置; D、带线导线与导线之间的接头必须绝缘包扎，带电导线必须绝缘良好; E、带电导线严禁搭、挂、压在脚手架或其它物体上; F、配电箱与开关箱应作名称、用途、分路标记;配电箱、开关箱应配锁并有专人负责; G、电箱内部及其周围临近区域不得有杂物、灌木和杂草等; H、室外用电严禁拉设使用花线，严禁使用铜线或其它金属线代替保险丝使用，严禁工人宿舍内乱拉电线、插座、烧电炉、电饭煲; I、电气装置应定期检修，检修时必须做到: ?、停电; ?、悬挂停电标志牌，挂接必要的接地; ?、由相应级别的专业电工检修; ?、检修人员应穿绝缘鞋和手套，使用电工绝缘工具; ?、有组织和专人统一指挥。 (6)、照明 A、在夜间施工或自然采光的场所、料具堆放场、道路、仓库、办公室、食堂、宿舍等设备一般照明、局部照明或混合照明; B、根据使用场所的环境条件选择相应的照明器; C、行灯电压不超过36V，灯具离地面高度底于2.4米等场所照明电压不大于36V。潮湿及易触及带电体场所照明电压不大于24V; D、根据需要设置警卫和红色信号照明和事故照明，其电源应设在施工现场电源总开关的前侧，并配备电源。 (7)、对各类用电人员进行安全用电基本知识培训。 2、安全用电组织措施 (1)、建立临时用电施工组织设计和安全用电技术措施的编制、审批制度，并建立相应的技术档案。 (2)、建立技术交底制度。 向专业电工、各类用电人员介绍临时用电施工组织设计和安全用电技术措施的总体意图、技术内容和注意事项，并应在技术交底文字资料上履行交底人和被交底人的签字手续，载明交底日期。 (3)、建立安全检测制度。从临时用电工程竣工开始，定期对临时用电工程进行检测，主要内容是:接地电阻值、电气设备绝缘电阻值、 23 漏电保护器动作参数等，以监视临时用电工程是否安全可靠，并作好检测记录。 (4)、建立电气维修制度。加强日常和定期维修工作，及时发现和消除隐患，并建立维修工作记录，记载维修时间、地点、内容、技术措施、处理结果、维修人员、验收人员等。 (5)、建筑工程竣工后，临时用电工程的拆除应有统一的组织和指挥，并须规定拆除时间、人员、程序、方法、注意事项和防护措施等。 (6)、建立安全检查和评估制度。施工管理部门和企业要按照JGJ59-88《建筑安全检查评分标准》定期对现场用电安全情况进行检查评估。 (7)、建立安全用电责任制，对临时用电工程各部位的操作、监护、维修分片、分块、分机、落实到人，并辅以必要的奖惩。 (8)、建立安全教育和培训制度。定期对专业电工和各类用电人员进行用电安全教育和培训，经过考核合格者持证上岗。禁止无证或随意串岗。 (9)、强化安全用电领导体制，改善用电技术队伍素质。 3、电气防火技术措施 (1)、合理配置、整定、更换各种保护电器，对电器和设备的过载、短路故障进行可靠的保护。 (2)、在电气装置和线路周围不堆放易燃、易爆和强腐蚀介质，不使用火源。 (3)、在电气装置相对集中的场所，如变电所、配电室、发电机室等配置绝缘灭火器材等，并禁止烟火。 (4)、加强电气设备相间和相一地间绝缘，防止闪烁。 (5)、合理设置防雷装置。 4、电气防火组织措施 (1)、建立易燃、易爆物和强腐蚀介质管理制度。 (2)、建立电气防火责任制，加强电气防火重点场所烟火管制，并设置禁止烟火标志。 (3)、建立电气防火教育制，经常进行电气防火知识教育和宣传，提高各类用电人员电气防火自觉性。 (4)、建立电气防火检查制，发现问题，及时处理。 (5)、强化电气防火领导体制，建立电气防火队伍。 4、现场消防措施 1、建立消防组织，设立防火小组和消防小分队，进行防火检查，及时消除火灾隐患。 2、编制防火技术措施。 3、按要求配置灭火器材并合理布置。 24 4、按施工平面布置做好临时消防龙头和消防水池。并时刻保证消防通道的畅通。 5、现场禁止使用明火，动火作业必须履行专职安全员审批制度。 6、工作区和生活区的照明、动力电路皆由专业电工按规定架设，任何人不得乱拉电线。 7、材料保管:对储存物品进行火灾危险性的分类并分开存放，各种气瓶等危险品应单独设库存放。 8、木工作业棚防火:采用阻燃材料搭设，电气设备应密封或采用防爆型，防止电线短路。用电设备过载运行，严禁在作业场所吸烟生火;配备足够的灭火器材。 9、电、气焊作业:焊割作业区与气瓶距离，与易燃易爆物品距离，乙炔发生器与氧气瓶距离都应大于安全规定距离，焊割设备上的安全附件要保证完整有效，作业前应有书面防火交底，作业时备有灭火器材，作业后清理燃物，切断电源、气源。 10、熬制沥青防火:应在指定地点(远离建筑物和易燃材料30M外)。 11、涂喷漆作业防火:保证场所通风良好;禁止焊割作业同时或同部位上下交叉进行;浸有涂料稀释剂的破布、棉纱、手套和工作服等应及时清除，防止堆放生热自燃。 12、项目部设立消防组织机构。 5、分项工程安全技术措施 1)、安全技术交底制度 (1)、工程开工前，应随同施工组织设计，向参加施工的职工认真进行安全技术措施的交底，使广大职工都知道，在什么时候、什么作业应当采取哪些措施，并说明其重要性。 (2)、每个单项工程开始前，必须重复交代单项工程的安全技术措施，坚决纠正只有编制者知道，施工者不知道的现象。 (3)、实行逐级安全技术交底制，开工前由技术负责人向全体职工进行交底，两个以上施工队或工种配合施工时，要按工程进度交叉作业的交底，班组长每天要向工人进行施工要求、作业环境的安全交底，在下达施工任务时，必须填写安全技术交底卡。 2)、分项工程安全技术措施 (1)、地面工程(细石混凝土、水泥砂浆)安全技术措施 A、操作时必须穿胶鞋，以免水泥浇脚。 B、晚上收光地面要有足够的照明。不得随便移动临时照明线，不准把衣物等挂在电线上。 (三)、 机械设备安全 25 1、 所有施工设备和机具在使用时均匀必需由专职人员负责进行检查、必要的试验和维修保养，确保状况良好。各技术工种必需经过培训经考核取得合格证，方可持证上岗操作，杜绝违章作业。 2、 驾驶、指挥人员必须持证上岗，必须按规程要求进行操作，驾驶员应作好例保和记录。 3、 各类安全(包括制动)装置的防护罩、盖等要齐全可靠。 4、 机械与输电线路(垂直、水平方向)须按规定保持距离。 5、 作业时，机械停放稳定，臂杆幅度指示器灵敏可靠。 6、 电缆线绝缘良好，不得有接头，不得乱拖乱拉。 7、 各类机械挂技术性能牌和上岗操作人员名单牌。 8、 必需严格定期保养制度，做好操作前、操作中的操作后设备的清洁润滑、坚固、调整和防腐工作。严禁机械设备超负荷使用、带病运转和作业运转中进行维修。 9、 机械设备夜间作业必需有充足的照明，夜间施工要有良好的照明设备。 四、文明施工保证措施 文明施工管理是企业施工生产经营的综合反映，我们将它贯穿于施工管理的全过程，并且争创“标准化施工现场”，采取相应的管理措施见下 (一)、 文明施工 1、 严格按照政府有关条件做到施工“标准化”、现场“景观化”。 2、 门前实行“三包”，保证现场各类材料堆码有序，现场排污水沟处于良好状态。 3、 施工工人操作地点和周围必需清洁、整齐，做到干活脚下清、活完场地净。 4、 钢筋分型号、规格、货架式堆放，并排牌标明规格，成型钢筋必需标明使用部分。 5、 施工机具要做到摆放整齐，机身保持整洁，标语编号明显，安全装置灵敏有效，机棚内上干净。 6、 运输各种材料、垃圾等有遮盖和防护措施，防止泥浆等随车带出场外，影响市容环境卫生。 (二)、 管理措施: (1) 贯彻文明施工的要求，推行现代管理方法，科学组织施工，做好施工现场的各项管理工作。 (2) 按照施工总平面布置图设置各项临时设施。堆放大宗材料、成品、半成品和机具设备，不得侵占场内道路及安全防护等设施。 (3) 施工现场设置明显的标牌，标明工程项目名称、建设单位、设计单位、施工单位、项目经理和施工现场总代表人的姓名、开、竣 26 工日期、施工许可证批准文号等。施工单位负责施工现场标牌的保护工作。 施工现场的主要管理要员在施工现场应当佩戴证明其身份的证卡。 (4) 施工现场的用电线路，用电设施的安装和使用必须符合安装规范和安全操作规程，严禁任意拉线接电。施工现场必须设有保证施工安全要求的夜间照明;危险潮湿场所的照明以及手持照明灯具，必须采用符合安全要求的电压。 (5) 施工机械按照施工总平面布置图规定的位置和线路设置，不得任意侵占场内道路。 (6) 保证施工现场道路畅通，排水系统处于良好的使用状态;保持场容场貌的整洁，随时清理建筑垃圾。在车辆，行人通行和地方施工，应当设置沟井坎穴覆盖物和施工标志。 (7) 施工现场的各种安全设施和劳动保护器具，必须定期进行检查和维护，及时消险隐患，保证其安全有效。 (8) 职工生活设施，符合卫生，通风，照明等要求。职工的膳食，饮水供应待应当符合卫生要求。 (9) 做好施工现场安全保卫工作，采取必要的防盗措施，在现场周边设立围护设施。非施工人员不得擅自进入施工现场。 (10) 严格依照《中华从民共和国消防条例》的规定，在施工现场建立和执行防火管理制度，设置符合消防要求的消防设施，并保持完好的备用状态。在容易发生火灾的地区施工或者储存，使用易燃易爆器材时，施工单位应当采取特殊的消防安全措施。 (11) 遵守国家有关环境保护的法律规定，采取措施控制施工现场的各种粉尘、废气、废水、固体废弃物以及噪声，振动对环境的污染和危害。 (12)、为了降低施工中噪声对环境的影响，采用如下措施: ?、通过统筹安排，合理计划，最大限度地减少夜间施工的时间和次数; (13) 搞好公共关系的协调工作。为最大限度地减少施工对周围环境的影响，我公司由专人负责公共关系的协调工程，随时听取有关方面对我施工的意见和建议，并在可能的情况下加以改正，满足有关部门的要求，使工程能顺利进行。 第四章、质量保证措施 (一) 工程质量目标 确保本工程质量达到合格等级。 (二) 工程质量管理体系 1、建立质量管理机构 27 公司坚决贯彻“百年大计、质量第一”的方针，牢固树立“预防为主”的思想。在施工过程中，通过各种形式，加强对参建职工的教育，不断提高工作责任心和质量意识，对质量问题坚决执行“三不放过”和“一票否决制度精心组织、精心施工确保工程质量达到合格率为100%。 为了充分发挥质量管理体系的作用，保证工程量合格，协调公司部门与项目经理部的关系，成立公司领导、项目有关人员参加的本工程施工质量控制小组控制本工程的质量。 2、建立工程质量保证体系 (1)、在全面熟悉施工图，充分领会设计意图的前提下，建立以项目副经理、项目工程师为首的质量保证体系，全面控制施工的项目的工程质量。 (2)、质量保证体系 我单位已通过了质量体系认证中心的换证复评工作，继续保持了ISO9001质量体系认证证书的有效性，这表明我单位具有完善的质量保证体系和较高的管理水平，具有保证工程质量的能力。 (3)、质量管理制度 1?我单位接受业主委托的当地质量监督站的质量监督及监理人员的全面监督，在施工中按监理规程中规定的报验程序执行，本着为用户提供优质工程和服务的原则，走质量兴业之路与各方一起同心协力，建好工程。 2?、我单位派驻此项目工地的质量检查人员均为经过培训，考核合格取得证书并且实践经验丰富的资深检查员，由单位授权从工程开工至竣工驻现场全方面直接监督检查，为严把质量关未经其签字认可，任何人不得擅自做主进入下道工序施工，把质量隐患消灭在萌芽状态。 3?、我单位制定了《监督检查管理规定》，规定单位定期进行质量全面检查，由分管领导或总工程师组织，各有关科室人员参加，并将其结果全单位下发通报，奖优罚劣，实行宏观监督;各有关业务人员参加，填写《质量体系运行监督检查记录》及《质量计划实施检查记录》，以求层层把关，及时发现问题，全面提高工作质量。 (三) 计量保证措施 1、 严格执行公司的讲师检测网络。 2、 材料由试验员和见证人共同取样送试验室检验，不合格不得使用。 3、 砼搅拌严格按配合比投料，砂、石要每盘过磅。 4、 现场设立砼快速测强点，由试验员负责操作。 28 5、 经纬仪、水准仪、地磅等计量工具由项目技改计量工长负责按规定送法检验单检校。 (四) 原材料设备保证措施 1、 对原材料质标准严格把关。材料员对原材料、成品半成品就先检验后收料，不合格的材料不准进场。 2、 原材料要具备出厂合格证或法定检验单出具的合格证明。钢筋、水泥还应注明出厂日期、批号、数量和使用部分，抄件应注明原件存放单位和抄件人并签章。 3、 对材料质量证明有怀疑或按规定需要复检的材料，应及时送检，未经检验合格后，不得使用。 4、 各种不同类型、不同型号的材料分类堆放整齐。水泥、钢筋在运输、存放时需保留标牌，按批量分类，并注意防久锈蚀和污染。 (五) 质量检测措施 一、 质量管理和保证 我单位制定了从开工到竣工各个阶段各个环节的工作制度(工作程序)， 多年来运行良好，对保证工程质量起到重要作用。 (1)、合同评审制度。在合同签订前(包括投标前)对合同条款进行细致的分析、研究，保证对合同要求准确理解和全面满足;在施工过程中，对一切与合同变更有关的信息进行及时反馈和处理，以保证履约能力为准则。 (2)、文件资料管理制度。对施工中涉及的一切文件和资料进行有效的控制，使管理工作日益规范。 (3)、物质采购管理制度。主要材料采购前通过调查货源、生产能力、质量水平、历史业绩抽取样品经验等手段确定供货单位或供货厂家，签订供货，并在使用过程中加强质量跟踪，及时发现质量问题，及时更换供应商。供应商的合格评定由授权的业务负责人或项目经理批准，其它人不擅自改变供应商进行采购。 (4)、材料标识和可追溯性制度。对进入施工现场的各种原材料、半成品认真清点分类堆放，并妥善保管材料员交来的随行记录，实行挂牌标识制度，标识清晰、醒目不易混淆，对结构材料、防水材料、结构构件、隐蔽工程使用新技术的部位，易出现质量通病的工序及质量计划中就界定关键和特殊工序等有可追溯性要求的产品及部位在各施工过程中，认真填写齐全的施工记录，而且建立施工前的人员资质、设备鉴定及质量控制方案，审核、监控记录的以实现其追溯性。项目部不定期抽查标识及各种记录，单位定期不定期组织检查。 (5)、施工准备管理制度。有计划、有目的系统性地做好工程开工前的各项准备工作，以保证工程按要求顺利开工和工程施工过程的质量等。 29 (6)、施工设备管理制度。项目部建立设备使用台帐，所有设备实行分级保养制，每班填写设备运转日记，每月编制保养计划，按规定时间进行各级各类保养，填写保养记录;焊机等特殊设备在使用前进行保养及设备能力鉴定，其操作者均是经培训合格后持证上岗。 (7)、施工过程管理制度。直接施工工序过程是形成工程质量的重要环节，也是施工现场质量管理的主要环节。主要包括技术交底制，三检制、技术复核制、关键、特殊过程控制等，其各控制方案具体要求为:总工程师向项目主任工程师书面交底，项目主任工程师对分部工程及此工程涉及到的“四新技术”应用向工号技术员、工长书面交底，工号技术员、工长将所有分项工程经项目主任工程师审批后，方可向相关施工作业班组成员进行技术交底，以上的三级交底均履行签字手续;施工中各分项工序实行由班组质量检查员进行的自检制，由操作者相互执行的互检制以及由工长、技术员、专职检查员、班组长参加的上、下工序交接检等“三检制”。没有班组自检记录的工序不予验收、结算、对焊接、砼浇筑等特殊工序施工前对焊接设备、搅拌设备、运输设备能力鉴定，对焊工等操作人员进行资格审查、考核，保证工序能力。 二、项目质量检测的具体措施 1、各级、各部门的质量职责 (1)、项目经理部的质量职责 ?、基本职责:强化监控，宏观控制服务现场，搞好协调。 ?、 加强对施工的监督检查，确保工程质量达到标准。 ?、 编制和落实职工教育和培训计划，不断提高职工队伍的整体素质。选聘合格的操作人员上岗。 ?、 重合同、守信誉，认真履行合同。负责提供物质保障和技术支持。 ?、 编制和实施项目的总体管理方案和施工计划，制定和落实技术、质量、工期、降低成本、安全生产、文明施工等各项科学管理措施，严格管理，合理计划，组织和调度进入现场的生产要素，保证项目质量、工期、效益、安全等目标的实现。 ?、 认真执行国家及企业的各项规章制度和专业管理办法，抓好各项基础管理工作。加强对工程质量的管理，对施工进行有效的监督、控制和管理。 (2)项目经理部质量管理的主要内容 ?、针对工程特点和合同要求，根据工程的《施工组织设计》和《质量策划》，设计项目质量目标。 ?、编制以质量为第一要素的项目《施工组织设计》。 30 ?、正确设立质量管理点。明确项目部内部质量职责分配，建立质量现任制度及考核度。 ?、严格执行各工序操作的工艺标准。根据国家施工及验收规范，对各分部分项工程质量进行检验、控制。 ?、完成质量记录，建立质量档案，做到质量可靠追踪。 ?、制定落实质量奖惩办法，实施质量一票否决权。 ?、对进行的设备、材料质量和数量今年性严格验收，妥善保管，合理利用。 ?、建立质量成本制度，提高建筑产品工序一次合格率。 (3)项目经理主任工程师质量职责 ?、对本工程质量管理和工程质量负全部技术责任，督促各级人员行使质量责任。 ?、组织贯彻技术规程、施工规范、质量标准、负责组织分级编制审定工程施工。 ?、组织设计、质量计划、特殊过程的施工方案。负责组织开展QC小组攻关活动。 组织科技发展规划和新材料、新工艺、新设备在本工程施工中的应用。 ?、不断改进施工方案的施工方法，采取有效技术措施，确保工期、进度、质量满足合同规定的要求。 ?、对检测、实验手段、管理制度，承担技术指导和领导责任。 ?、重视用户和投诉处理，负责组织不合格工程的原因分析、评定和纠正，预防措施的制定、实施;主持重点质量事故和重要质量问题的调查处理与改进。 (4 )作业队长质量职责 ?、负责施工准备工作，审核施工组织计划及开复工报告。 ?、检查测量情况，对工程组织测量复测。 ?、负责测量、实验器具的使用情况检查、配置能力分析。 ?、对工程质量不合格品进行处理。 ?、负责施工管理的各项技术交底工作，负责现场工作人员材料、机械、环境、施工方法的检查工作。 ?、参加质量策划，安排施工顺序，负责施工组织设计的实施工作。 ?、下达雨季施工措施，并组织实施。 ?、负责采购文件的检查指导工作。 ?、负责半成品、结构件的标识管理、检查工作，负责标识文件制定。 ?、负责关键和特殊过程的控制管理工作，收集各种资料进行过程能力的认可。 31 ?、负责各种质量记录的管理工作。 ?、负责产品交付后的服务工作，进行质量回访、投诉、处理、调查客户意见的组织管理工作。 (5)技术员质量职责 ?、负责图纸会审和技术交底工作。竣工文件的指导审查归档工作。 ?、负责实施性施工组织设计、安全设计、各种技术组织措施的制定。 ?、负责工程技术总结、科研和新工艺、新材料的收集、整理、总结。 ?、负责《项目质量计划》的管理、修订、更改和执行工作。 ?、负责施工管理和技术管理中的作业指导编制、更改、实施工作。 ?、负责统计技术的运用管理和指导工作。 (6)资料员质量职责 ?、负责施工技术管理文件、资料、图集、标准、空白表的打印、建帐、登记、造册、保管、标识和收发工作。 ?、负责竣工文件收集、整理和建挡工作。 ?、负责合理化建议和科研项目的资料整理。 ?、负责描图、晒图及各种台帐的建立工作。 (7)实验员质量职责 ?、负责原材料、构、配件和施工过程中的检验、实验工作，保管实验报告。对实验不合格的产品，及时上报总工、质检工程师，分析原因，通知施工班组改正。 ?、负责纠正措施所必须的实验工作，保管实验报告。 ?、负责实验设备的校准、标识、保养与管理。 (8)质检员质量职责 ?、负责项目施工的质量检查记录。经常分析质量状况，掌握工程的质量动态。下发不合格的整改通知单。 ?、按规定检查各工种操作质量原始记录，及时向上级反馈质量信息。 ?、负责本单位检验器具的建档、定期校准、标识工作。 ?、掌握质量管理中常用的统计方法、计算、分析、运用。 ?、负责施工项目上手特殊工种人员上岗工作。 ?、参与质量事故的调查处理工作。 ?、通过工程项目经理部的协调指挥，实施动作，严格ISO9001程序进行质量管理，这是质量保证措施的关键所在。 1、 施工准备进程的质量检测措施 (1)、按优化的施工组织设计和方案准备工作。 32 (2)、搞好图纸会审和技术培训工作。对推广应用的新技术、新工艺组织有关施工人员认真学习。 (3)、按施工组织设计和施工总平面布置进行场地布置和建点工作。经理部全部管理人员配齐并履行自己的质量职责。 (4)、做技术交底工作，总的施工控制是各分项、分部工程都做好技术交底工作，使各级管理人员和施工做到心中有数。 (5)、对于砼、模板、钢筋绑扎等关键过程加强控制，制订好《作业指导书》，推行全面质量管理，分别建立QC小组，各小组均制定自己管理目标，以便遵照执行和检查，开展群众性的攻关活动。 (6)、材料采购力求货比三家择优选用，建立合格分供方档案备查，对不合格产品杜绝使用，进场材料不仅有出厂合格证明文件，并有专人、专帐、专库管理。建立相应的管理制度。 (7)、根据施工组织设计，合理选择施工机械，搞好维护检修工作，保证机械设备的良好状态。本工程选用机械设备性能优良、运转良好的设备。 (8)、做好各工程、各工序的接口工作，根据施工总进度计划，明确各工程的开工时间和要求完成时间，并明确其质量和施工技术要领。 三.施工过程中的质量检测措施 (1)、质量管理点的原则:在工期、工序交接、测量放线、砼模板、钢筋连接、设备安装等等管理点设立质量管理小组，由专人控制各管理点的施工质量。 (2)、根据各管理小组工作特性，确定各小组的质量目标和质量管理措施，明确各自的质量责任。 (3)、建立高效灵敏的质量信息反馈系统。由专职质量检查员、技术人员作为中心负责搜集、整理和传递质量动态信息给决策机构。决策机构对质量异常情况迅速作出反应，并将新的指令信息给执行机构。 (4)、在施工中，对于每道工序，每个操作人员，做到质量工作的“三落实”即: ?、施工前，施工管理人员对施工班组进行书面技术交底，使每个操作人员明确操作要点及质量要求。 ?、施工过程中施工管理人员随时检查指导施工，将日常检查和月检查结合进行，使质量问题消灭在萌芽之中。 ?、每道工序施工结束后，及时组织质量检查评比，进行工序交接。上道工序质量不合格，下道工序不开始，并根据检查结果对施工班组及操作人员进行奖优罚劣，强化施工人员质量意识。 33 (5)、采用质量预控法中的因果分析图，使质量处于良好的受控状态。 (6)、严格按照PDCA循环过程，有秩序开展质量管理小组活动，其步骤是: 找出问题?分析问题?找出主要影响因素?拟定措施?认真执行措施?检查效果?总结经验纳入标准?处理遗留问题，转入下期循环?全面推行标准化作业，经标准化统一现场生产、管理 (7)、发挥我单位计量、试验、科研方面的优势，并用于现场发挥成效。 ?、计量工作的重点放在各种配合比的设计、砂、石计量方面，并保证所有测量、计量器具的准确性。 ?、工程试验把好原材料的取样、检验关，严格配合比设计，并及时注意施工时的调整，按规定做好试件，按要求进行试件的养护、试压。 ?、将我单位拥有的科研成果应用于本工程的施工中，并根据施工实际，开发新的施工技术。 (8)、执行设计文件审核制度。分项工程进入施工状态前由总工程师负责组织人员复审，发现问题及时报业主协商解决。
