根据铁道部工管中心《关于开展铁路建设项目大型机械设备施工安全隐患排查专项整治活动的通知》(工管质安电〔2012〕95号)要求，公司决定自7月27日-8月31日，在全线开展一次对架桥机等大型机械设备及挂篮施工安全隐患排查专项整治活动，现将有关要求通知如下: 一、成立组织机构 公司成立安全专项整治领导小组和实施小组。 1(领导小组 组 长:李贵祥 副组长:靳忠、范振平 成 员:安质部部长、工程部部长、区段指挥部指挥长 2(实施小组 成立两个现场实施小组。 第一组组 长:靳 忠 组 员:董福田、刘 辉、时均新、房爱勇 陈庆友、肖 鹏、张茂华、黄永红。 负责检查站前施工1-8标。 第二组组 长:范振平 组 员:刘江川、时均新、宋四宝、徐思智 阴 凡、范旭辉、马永红。 负责检查站前施工9-15标。 二、管理方面检查的重点内容 1(监理、施工单位是否制定了大型机械设备安全;监理单位是否建立总监定期现场检查，是否明确分管副总监、专业监理工程师，是否坚持现场跟班作业;施工单位是否明确分管领导并坚持现场跟班作业。 2.施工单位是否编制大型施工机械设备专项施工，专项施工方案是否按程序审批。 3.施工单位设备管理部门是否对大型施工机械设备进行了自检，关键部位是否请制造厂商到现场进行了检查;监理单位是否对大型机械设备管理程序进行了全面核查落实。 4.设备使用单位是否按要求向当地技术质量监督部门报检，并取得检验合格证。 5.特种设备是否符合《特种设备安全监察》中规定的持证作业等要求。 6.施工单位是否对机械操作人员及相关施工人员进行了安全教育培训。 7.大型机械设备是否满足相关的设计、制造、运输、组装调试等技术要求及安全管理规定。 8.大型施工机械设备是否按规定进行大修、中修、定期维修及日常保养，是否对起重设备的钢丝绳、吊具、承重轴及整体稳定性等关键要素加强安全管理。 9.大型机械设备制造厂商资质是否符合要求，制造用材料或零配件是否合格，型式试验和使用前验收是否符合要求。 10(施工单位是否对机械设备操作人员进行了书面安全技术交底。 11(施工单位是否按机械使用说明书和安全技术规程，制定了机械各部位检查标准和方法要求，是否明确到各岗位，是否在班前或交接班前进行了检查，机长是否对各部件的检查结果及整机性能状态进行了确认，跟班领导是否进行了确认。 12(大检查活动开展情况。是否按铁道部和公司文件要求开展了安全大检查活动。 13(风险管理情况。是否按公司风险管理实施细则要求 进行了风险研判，是否列出风险清单，是否将责任落实到个人，卡控措施是否具有可操作性，是否进行了考核和责任追究。 三、主要设备检查的重点内容 1.检查主要设备:(1)架桥机、(2)提梁机、(3)运梁车、(4)移动模架、(5)龙门吊、(6)塔式起重机、(7)轮胎式起重机、(8)卷扬机，计8项。对其它机械设备及挂篮施工，要求施工、监理单位也必须全面检查。 2.检查的重点内容:上述8项主要设备检查重点内容计104条，详细内容见附件表1--表9。 四、活动时间安排 本次安全隐患排查专项整治活动分为三个阶段: 第一阶段:2012年7月27日-8月7日，为各单位自查自纠阶段。 第二阶段:2012年8月8日-8月15日，为公司全面督导检查阶段。 第三阶段:2012年8月16日-8月31日，为整改验收和总结阶段。 五、有关要求 1.高度重视，认真布置。 各单位主要领导要高度重视，真正吸取“7.19”韩家屯特大桥架桥机倾倒事故教训，各施工单位指挥长、监理单位总 监或中方常务副总监要亲自带队，认真布置，并组织开展好大型机械设备施工安全隐患排查专项整治活动。 2.严格安全风险评估管理。 各单位要依据公司风险管理实施细则，组织对大型机械设备作业的每一个环节进行动态安全风险评估，每月按时确认各种大型机械设备安全风险清单，并针对存在的风险采取可行的防范措施，防范措施要责任到人，要进行严格考核和责任追究。 3.认真检查，问闭合。 要求各施工单位结合铁道部“打非治违”活动，对大型机械设备全面进行排查，做到不留任何死角，对存在安全隐患的施工设备，必须立即停止使用，不经安全评估合格不得使用。要求各检查人员，按照确定的检查重点，逐项认真检查，对检查出的问题及时下发书面整改通知单并跟踪整改闭合，书面闭合资料报公司安质部。 4.建立台帐，认真总结。 各施工单位要落实专人负责大型机械设备的管理，定期组织对大型机械设备的检查，建立专门的大型机械设备管理台帐和问题库台帐。对此次安全隐患排查专项整治活动情况认真总结，要求施工、监理单位、公司两个检查组书面总结于8月25日之前报公司安质部，公司各检查组分别下发检查通报。 5.防患未然，建立长效机制。
