1、建立和健全以安全生产责任制为中心的各项安全#管理#和组织机构，是保障安全生产的重要组织手段。由于安全问题贯穿于生产的全过程，凡是与生产全过程有关的部门和人员，都对保证生产安全负有相应责任，因此健立各级人员安全生产制及组织机构，是保障安全生产的前提条件。 本工程安全生产组织机构图见下图: 项目安全总负责 秦富强 生产安全负责人 技术负责人 方卫平 高祥云 施工生产组 技术质量组 办公室 水电班 木工班 钢筋班 瓦工班 架子工 张友武 吴孙成 刘学伦 李光兵 娄尔清 全体操作人员 4 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 各级安全责任制详见附件(一) 2、技术人员在工程施工前应详细编制各工种安全技术操作规程，并经公司领导审核批准，专职安全员罗军、吴尔福、涂小兵及各工种负责人负责在施工过程中组织监督实施。 3、本工程安全管理目标为:死亡重伤率为0，轻伤率控制在0.3%以内。杜绝火灾机械事故。文明施工确保达到“合肥市安全文明示范工地”。应将安全管理目标进行层层分解并与部门和班组长签订包保责任书，制定安全生产责任考核制度，经理将安全生产责任考核纳入经济承包制度中。 4、针对广福花园B地块1#,13#楼、幼儿园及地下车库工程的特性，编制专项安全文明施工组织设计汇入“广福花园B地块1#,13#楼、幼儿园及地下车库工程施工组织总设计”中，并经公司总工程师及工程部审核批准。 5、在分部分项工程施工前，应针对不同分项工程的特点，有针对性的编制分部(分项)工程安全技术措施，进行详细的书面安全技术交底，交底内容应全面，签字手续要齐全。 6、建立安全检查制度。确定每月20日为项目部安全大检查日，由项目经理或副经理带队，组织项目部各部门，对照国家安全检查评分标准进行检查，并将检查出的安全隐患及时反馈。施工部门定人、定时、定措施组织整改落实。确定每周五为各班组安全检查日，由项目部安全员组织各班组长、班组安全员结合班组施工安全情况进行检查，对检查出的安全隐患及时整改。每次安全检查均应有详细的检查记录。 7、制定安全教育制度。安全教育分为思想教育、知识教育、安全技术和技能教育、事故教育、法制教育。对新工人进行上岗前的三级教育，对变换工种的工人应进行转岗教育，对施工管理人员和专职安全员应按规定进行年度培训和考核制度。 安全生产教育的内容详见附件(二) 8、建立班组日常安全教育制度，每周一为安全活动日教育，每日班前进行安全讲话，并做好记录。 9、建立特种作业持证上岗制度。特种作业包括:电工作业，电焊和气焊作业，机动车辆作业，建筑登高架设作业及其它符合特种作业定义的作业。 5 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 所有从事特殊作业的人员均应持证方可上岗。 10、建立工伤事故报告制度。伤亡事故发生后，负伤者或者现场有关人员应立即直接或逐级报告工程项目(企业)的负责人。工程项目(企业)负责人接到重伤、死亡或重大伤亡事故报告后，应迅速赶到事故现场，指挥抢救受伤人员，同时立即报告企业的上级主管部门和企业所在地市、县级建设行业主管部门及检察、劳动部门。与此同时，应组成事故调查组对事故原因进行调查，并按规定对事故进行处理，建立工伤事故档案。 11、施工现场按规定设立安全标志总平面布置图，并按平面图设置安全标志。 12、建立安全例会制度，项目经理部每月20日召开一次安全例会。 四、文明施工方案 1、工地现场入口处挂设工程概况，并设置门楼，施工现场内的围挡材料采用砖砌围墙，并且坚固、稳定、整齐、美观。 2、施工现场实施封闭管理。在现场主要出入口设置门卫房，建立门卫登记制度。同时进入施工现场人员均应佩戴工作卡。现场出入口设置企业标志，门楼并插旗帜。现场大门口挂“五牌一图”，标牌规范、整齐，设安全标语，宣传栏、读报栏、黑板报等设施。 3、施工现场主要地面应做硬化地面，现场设3.5米宽的硬化主干道，保持道路畅通。现场道路两侧，搅拌场地，房前屋后等均应设置排水沟，现场出口设置车辆冲洗台，施工污水汇入沉淀池再引入窨井排出场外，做到工地无积水，保持场容场貌整洁。现场主要位置设置禁烟标牌和吸烟室，楼内严禁吸烟。现场临时垃圾堆放点设置围挡材料。 4、施工现场建筑材料垃圾、构件、机具严格按总平面图布置地点堆放，并对名称、品种、规格等进行标识，并做到按品种、规格统一堆放整齐。对易燃易爆物品应做到分类单独存放。 5、施工现场在建工程内严禁住宿，现场生活区和办公区与施工区要相 6 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 对独立分开。生活区内周围环境应保护干净、整洁。床铺排放有序，生活用品放置整齐，宿舍内应采取保暖与防煤气中毒的措施和消暑与防蚊虫叮咬措施。 6、现场设置职工活动室，供职工学习和娱乐用。同时建立施工现场保卫制度，责任分解到人，现场严禁偷盗，打架现象的发生。 7、生活区内设置食堂与厕所。食堂应取得卫生许可证方可开业，同时应建立卫生制度，确保职工饮食饮水卫生。厕所设置水冲式厕所，厕所内墙面贴瓷砖，地面贴地砖，同时设专人打扫厕所，保持厕所卫生。现场生活区垃圾及时清理，设置专用垃圾容器，专人管理。 8、夜间施工应取得许可证后方可施工，同时应采取有效措施控制噪音，防止施工扰民，建立施工不扰民制度，现场严禁焚烧有毒有害物质。 五、脚手架施工方案及安全措施 (一)、脚手架施工方案 本工程为框架剪力墙结构，外墙脚手架采用双排钢管脚手架，脚手架主要技术参数如下: (1)立杆参数 钢 管:Φ48*3.5 立杆间距:0.85m 墙距(里排):20cm 连接方式:接头扣件 (2) 大横杆参数 钢 管:Φ48*3.5 上下步距:1.8m 连接方式:接头扣件 (3) 斜撑、剪刀撑参数 钢 管: Φ48*3.5 7 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 连续间距: 10m 与地面角度: 45度 (二)脚手架搭设与拆除 1、 施工准备 (1) 由项目副经理向架设脚手架人员进行技术交底。 (2) 对钢管、扣件、脚手板等进行检查，不使用不合格产品。 (3) 清除搭设场地杂物，平整搭设场地，并使排水畅通。 2、 搭设 (1) 脚手架必须配合施工进度搭设，一次搭设高度不应超过相邻连 墙体以上二步。 (2) 底座、垫板应准确地放在定位线上。 (3) 立杆不得混合使用不同外径的钢管。 (4) 相邻立杆的对接扣件不得在同一高度内。 (5) 在封闭型脚手架的同一步中，纵向水平杆四周交圈，用直角扣 件与内外角部立杆固定。 (6) 剪力撑，横向斜撑应随立杆、水平杆同步搭设。 (7) 脚手板应铺满、铺稳，离开墙面120-150MM。 3、 拆除 (1) 拆除前应清除脚手架上杂物及地面障碍物。 (2) 拆除作业必须由上而下逐层进行，严禁上下同时作业。 (3) 连墙体必须随脚手架逐层拆除。 (4) 各构配件严禁抛掷到地面。 (二)脚手架安全措施 1、作业层距楼地面高度?1.5m时，在其外侧边缘设置离作业层高1.1m高的栏杆和180mm高的挡脚板，且栏杆间净空高度应?0.8m。 2、外脚手架外侧采用密目安全网全封闭，脚手架内侧立杆与墙面间的缝隙在作业层采用安全网全封闭围护。 3、对通道口或者有行人通过的出入口，必须搭设通道防护棚，防护棚的搭设按合肥市的有关规定执行，采用双层竹笆，间距不小于400mm(如图)。 8 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 防护棚 800 3000 2000 2000 4、上下脚手架的斜道应设置扶手栏杆，或其它安全防护措施，并清除通道中的障碍，确保上下人员安全，上下通道设置防滑条。 5、装设连墙杆或其它撑拉杆件时，应注意掌握的松紧程度，避免引起杆件和整架的显著变形。 6、工人在架上进行搭设作业时，作业面上应满铺设竹笆脚手板并予以临时固定，工人必须带安全帽和正确系挂安全带。 27、脚手架使用荷载每m不大于300kg，严禁超载施工。 六、基坑防护施工方案 根据工程实际情况，基坑开挖拟采用1 :0.75放坡。 土方开挖施工时，应注意以下几点安全措施: 1、开挖边坡土方时，严禁切割坡脚以防导致边坡失稳。开挖后要在基坑上下采用明沟排水和截水。 2、机械行驶道路应平整、坚实，土方开挖应分层进行，防止塌方、溜坡造成机械倾翻事故。 3、机械施工区内禁止无关人员进入场内，挖掘机回转半径范围内不得站人或进行其它作业。 4、挖掘机操作和汽车装土行驶要听从现场指挥，所有车辆应严格按规 9 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 定的路线开行，防止撞车。 5、夜间作业时，机上及工作地点必须有充足的照明设施，在危险地段应设置明显的警示标志和护栏。 七、模板工程施工方案 (一)、模板施工工艺 模板支设顺序:检查模板平整度 ? 侧模板安装就位 ? 安装纵横管楞 ?穿对拉螺栓 ? 支设支撑 ? 调整垂直度 ? 紧固支撑。 梁模板支设顺序:搭设梁模脚手架 ? 铺设梁底模板 ? 安装梁侧模 ? 安装梁侧模板钢管楞 ? 复核梁模尺寸标高 ? 与相邻柱的连接固定。 楼面模板支设顺序:搭设模板脚手架 ? 铺设楞木(50×100木方间距300mm) ? 铺设组装模板 ? 复核模板平整度及标高。 (二)、模板及支撑的设计依据 模板及支撑设计时应计算以下荷载:模板及支撑自重、新浇砼重量、钢筋重量、施工人员与施工设备重量、振捣砼时产生的荷载、新浇砼对模板的侧压力。 模板及支撑设计、安装必须符合如下规定:保证工程结构和构件的形状、尺寸和位置的正确、具有足够的强度、刚度和稳定性、构造简单、装拆方便、模板严密不漏浆。 本工程上所有模板均用新胶合板与50×100木方制成定型模板，每块模板在制作之前均需出图，并附有相应的计算书，经现场技术负责人审核后方能制作。没有模板制作详图、计算书及技术负责人的审核不得制作。 (三)、施工要求 所有模板必须进行翻样，并经审核后制作。模板采用胶合板，其板面必须刷脱模剂，并按翻样要求进行制作。其模板接缝用特种胶封闭。在有防水 10 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 要求处，穿心螺栓上设止水环，内外模板与内、外排架固定牢固。 现浇板梁模也采用胶合板，经翻样制作，钢管排加上先铺50×100木方，再铺胶合板，便于固定胶合板，防止挠曲。 (四)、墙柱模板支撑 本工程墙柱采用12mm厚竹胶合板配制，竖向采用50 100木方背楞，横向采用ф48×35钢管柱箍，以保证砼的施工质量，且模板不被破坏。墙柱箍间距，从上往下:第1道距梁底200，第2-4道间距500，以下为400，最后一道距地面150-200。 (五)、梁、板模板 模板拟采用18mm厚胶合板，表面光洁，硬度好，周转次数高，砼成型质量好。采用钢管脚手架支撑体系，(钢管立杆间距800 800)，此支撑体系具有多功能、效率高、承载力大，安装可靠。便于管理等特点。梁板底模顶板搁栅采用50 100木方，搁栅间距300mm，100 100木方作搁栅托梁，间距1.2m。梁模板采用18mm厚胶合板。梁高超过1000mm的大梁在施工前要作专项施工模板设计，本工程跨度一般超过4m，故梁按规定必须起拱，起拱值不少于2%。 (六)、特殊部位模板 所有特殊模板都必须先作专项设计并由木工翻样绘制施工图，翻样图以设计和图纸会审纪要及修改通知单为依据。 1、楼梯模板 楼梯模板一般比较复杂，施工前根据实际层高放样，先安装休息平台梁模板，再安装楼梯模板斜楞，然后铺设楼梯底模。安装外帮侧模和踏步模板。安装模板时要特别注意斜向支柱(斜撑)的固定，防止浇筑砼时模板移动。楼梯段模板组装图如下: 11 高度?800mm的梁在梁高1/2处要设拉结筋(对拉螺栓)一道，拉结间距沿纵向600-800mm。拉结筋(对拉螺栓)采用Φ12,Φ14的钢筋制作。 (七)、预理件和预留孔洞的设置 1、竖向构件预理件的留置 采用绑扎固定的方法:用铁丝将预理件锚脚与钢筋架绑扎在一起(如下图所示)。为了防止预理件位移，锚脚尽量长一些。 12 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 钢筋背枋 1(严格控制柱顶标高(误差不超过?10mm)，模板安装必须严格按模板设计施工，模板相邻高差不超过3mm，所有节点必须逐个检查是否拧牢卡紧。 2(模板在使用前，均刷隔离剂，使用后及时清理归堆备用。 3(严格控制模板和立柱拆除时间，顶模板待砼强度达75,以上方可拆除。悬挑结构要达到100%设计强度才能拆模。 4(模板的装拆和运输应轻放，严禁摔砸，严格控制施工荷载，上料要分散堆放，在支撑过程中必须先将一个网格的水平支撑及斜撑安装好的，再逐渐向外安装，以保证支撑系统在安装过程中的稳定性。 5(模板钢筋施工完毕，浇捣砼前，用工业用吸尘器将模板内的杂质吸尽。 6(模板拼缝处要逐个处理，为保证模板拆除后砼表面能达到清水砼效果，所有模板的拼缝处均用双海棉嵌缝胶粘贴密实，防止梁、柱阳角处因漏浆引起的漏筋和不密实现象。 7(每层楼的梁、柱模板同时支撑好，以免因二次支模造成砼漏浆和表面不平正现象。 8(模板的拆除实行拆除通知单制度，所有的部位在砼浇捣后，都有专人负责将统一制作的标牌悬挂于该处最醒目的地方，标牌上标明此处砼的浇捣时间、砼的强度等级、责任人、以及侧模和底模的拆除时间。 16 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 八、“三宝”及“四口”防护措施 1、施工现场安全防护用品必须按合肥市建设安全技术协会颁发的准用证名录，由施工单位验证后购买，确保安全防护用品符合国家有关标准要求。 2、进入施工现场的所有人员均应佩戴合格的安全帽，且佩带方法应正确，对于不按规定配戴安全帽和不带安全帽的，一经发现，给予每人每次50元的罚款。 3、本工程外脚手架采用钢管脚手架，外侧采用满挂密目式安全立网封闭，安全网的规格、材质应符合有关规定要求，且安全网应取得合肥市建筑安全监督管理部门的准用证和合格证。 4、高空作业必须正确系挂安全带，安全带系挂时应高挂低用，防止摆动碰撞，绳子不能打结，钩子要挂在连接环上。在攀登和悬挂作业中，应有牢靠的挂钩处，严禁只在腰间佩挂而没有在固定的设施上栓挂钩环。 1955.5 1、施工现场临时用电安全措施: ?对于外电保护，要有一定的安全距离，防护要封闭严密，符合要求，对于工作按地与保护零线的设置要符合要求。 ?配电箱与开关箱要符合“三级配电”“两级保护”的防护措施，实行“一机、一闸、一 漏、一箱”且配电防护措施符合要求。 ?现场照明的专用回路应有漏电保护，潮湿作业必须使用36V以下的安全电压，室内线管及灯具的安装高度在2.4m以上，且用安全电压供电。 ?配电线路中电杆、横杆、架空线路必须符合要求，电线应无老化、无包扎，电缆架设或埋地也必须符合要求。 ?闸具、熔断器参数与设备容量相匹配，安装应符合要求，严禁用金属丝代替熔丝。变配电容量也应符合安全规定。 ?配备专人负责用电档案的管理工作。收集专项用电施工组织设计，地极电阻值摇测记录，电工巡视维修记录等文件。 2、用电量计算 施工用电量计算P=1.1(K1?Pc+K2?Pa+K3?Pb) 其中 本工程施工现场用电贯彻执行"安全第一，预防为主，综合治理"的方针，确保在施工现场用电中的人身安全和设备安全，使施工现场供用电设施的设计、施工、运行及维护做到安全可靠。 18 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 (1)分级配电箱、接线盒的位置选择要求:靠近电源，便于线路的引入和引出;保证不受雨水冲浸，不积水，地面排水坡度不小于0.5%;避开易燃易爆危险地段。 (2)理设电缆线路的要求:理设电缆线路的沟槽采用砖砌，尺寸0.3×0.8m，沟底铺砂100厚;在同一挡距内，一根导线的接头不得多于1个，同一条线路在同一档距内接头不应超过2个;进入配电房的电缆沟或电缆管，在电缆敷设完成后将管封实。 (3)接地保护 大型机械设备的金属包壳采取可靠的接地保护;电器设备的工作零线与保护接地线须分开，保护零线上严禁设开关或熔断器。 用电设备接地采用并联接地方式，严禁串联接地或接入零线。 接地线采用焊接、压接、螺栓连接或其他可靠方法连接，严禁缠绕或勾挂连接。 (4)防雷保护 施工现场塔吊、外脚手架、提升井架等20m以上的设备，均装防雷保护装置; (5)配电箱和开关箱 配电箱和开关箱应安装牢固;地面上所设的配电箱和开关箱，位置需高出地面，附近不可堆杂物。配电箱和开关箱的进线口和出线口设于侧面底面，电源引线设防水弯头;照明和动力合用同一配电箱时，应分别配闸刀。 (6)熔断器和插座 熔断器和插座规格应满足被保护的线路所需的要求，严禁使用不合格格熔断器，严禁使用铜丝代替熔丝;熔体溶断后，必须查找原因，经复核无误后方可更换，装好后方可送电;插销和插座必须配套使用。 (7)移动式电动工具和手持电动工具 长期停用或新领用的移动式电动工具和手持式工具在使用以前应进行检查有漏电情况发生;使用的移动式电动工具当采用插座连接，插座接头应无损伤、无裂纹，而且绝缘良好。使用的移动式电动工具，人员因故离开现场暂停工作或突然停电时，应拉开电源;使用的移动式电动工具应加装高灵敏 19 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 性漏电保护装置;使用的移动式电动工具，其电缆线应按电器规格和使用环境要求选用，电缆避开热源，不得在地上拖拉; (8)电焊机 电焊机应按区域或标高集中布置;室外电焊机应设置在干燥场所，并设遮棚;电焊机外壳必须可靠接地;电焊机的裸露导电部分和转动部分应装安全护罩;电焊把钳绝缘必须良好;电焊机二次引出线长度不宜大于30m。 (9)塔机 塔机的电源电缆应经常检查，必要时应设专人维护;未经有关人员批准，起重机上的电器设备和接线方式不得改动;电器设备需定期检查，起吊工作时不得进行电器检修;起重机的检验和试运行，必须取得专业人员的配合;塔吊的防雷和接地，应按规范要求进行，采用自然接地体时，应保证有良好的电气通路。 (10)照明 照明线路应布置整齐，位置相对固定;室内固定式照明灯具高度不得小于2.5m，室外安装的照明灯具不得低于3m。安装在露天工作场所的照明灯具应选用防水性灯头;室内照明线除橡皮套软电缆和塑料护套线外，均应分开固定于绝缘体上，穿墙线应设绝缘套管。照明线路不得接触潮湿地面，不得接近热源和直接挂于金属件上;照明开关应控制相线，当采用螺口灯头时，相线应接在中心触头上;照明灯具于易燃物之间，应按规定保持一定的安全距离，当间距不够时，应采取有效的隔离措施。 十、垂直运输机械防护措施 (一) 本工程在主体施工阶段拟采用塔吊，装修阶段采用附墙吊，其的安装、使用、拆除均应符合以下规定: 1、塔吊、附墙吊必须有建筑安全监督管理部门的颁发的准用证，方可使用。 2、按照规范要求设置附墙吊定型化吊篮停靠装置、超高限位装置。 3、附墙吊的卸料平台两侧均应设置防护栏杆与踢脚板，并用安全网围 20 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 护平台，拉设应牢固严密。出料口设有防护扶手。吊篮设自动安全门，提升时使用双根钢丝绳，严禁吊篮乘人上下。 4、塔吊、附墙吊的架体安装、拆除时应制定详细的施工方案。其基础要牢固，符合要求，卷扬机应搭设操作棚安装地锚，卷筒上设有防止钢丝绳滑脱保险装置，同时设置与钢丝绳匹配的滑轮，架体应安装避雷装置。 5、塔吊、附墙吊上应设置合理、可靠的联络信号。 6、架体安装完毕后，经公司安质部及相关部门验收、办理验收手续，所有验收人员均应签字。 (二) 吊装施工方案 1、物料在吊篮内应均匀分布，不得超出吊篮。当长料在吊篮内立放时，应采取防滚落措施，散料应装箱或袋笼。严禁超载使用。 2、严禁人员攀登、穿越提升机架体和乘吊篮上下。 3、提升机作业时设固定人员指挥。作业时无论任何人发出紧急停车信号，均应立即执行。 4、闭合主电源前或作业中突然停电时，应将所有开关扳回零位。 5、发现安全装置失灵时，应立即停机修复。 6、使用中要经常检查钢丝绳，滑轮工作情况。如发现磨损严重，必须即时更换。 7、作业后，将吊篮降至地面，各控制开关扳至零位，切断主电源，锁好闸箱。 十一、施工机具安全防护措施 1、所有施工机械、机具安装后均应有验收合格手续后再使用，且所有用电设施均应设置漏电保护器与保护接零。 2、平刨、圆盘锯等均应设置护手等安全装置。传动部位均应设置防护罩或防护档板，严禁使用平刨与圆盘锯合用一台电机的多功能木工机具，无人操作时应切断电源。 3、使用手持电动机具，要按规定穿戴绝缘用品，不得随意接长电源线或 21 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 更换插头。 4、电焊机应设置二次空载降压保护器或防触电装置，焊接线接头和绝缘老化应符合要求，电源应使用自动开关，电焊机应设置防护雨罩。 5、搅拌机的离合器，制动器，钢丝绳安装达到要求，操作手柄有保险装置，传动部分设防护罩，搅拌机应搭设防雨棚作业台，搅拌机还应设置水量自动计量装置。 6、各种气瓶应按规定设置标准色标，气瓶间的间距应大于5m，距明火距离应大于10m，若小于10m，须采取隔离措施，气瓶的使用与存放应符合安全规定且须设防震圈和防护帽。 7、潜水泵应设置保护装置，使用应符合有关要求的规定。 十二、高处作业防护措施 1、凡是坠落高度基准面2m以上(含2m)有可能坠落的高处进行的作业均称为高空作业 ，高空作业应符合以下基本规定: ?高处作业中的安全标志、工具、仪表、电气设施和各种设备，必须在施工前加以检查，确认其完好，方能投入使用。 ?攀登和悬空高处作业人员以及搭设高处作业安全设施的人员，必须经专业技术培训及专业考试合格，持证上岗，并必须定期进行体格检查。 ?施工中对高处作业的安全技术设施、发现有缺陷和隐患时，必须及时解决，危及人身安全时，必须停止作业。 ?施工作业场所有坠落可能的杆件，应一律先行撤除或加以固定，高处作业中所用的物料均应堆放平稳，不妨碍通行与装卸。工具应随手放入工具袋，作业中的走道、通道板和登高用具，应随时清扫干净。拆除下来的构件及余料和废料应及时清理运走，不得任意乱置或向下丢弃，传递物件禁止抛掷。 ?雨雾天进行高处作业时，必须采取可靠的防滑、防漏措施。凡水、冰、霜、雪均应及时清除。遇有六级以上强风浓雾等恶劣气候，不得进行露天攀登与悬空高处作业。暴风雪及台风暴雨后，应对高处作业安全设施逐一加以 22 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 检查，发现有松动变形、损坏或脱落等现象，应立即修理完善。 ?因作业必需临时拆除或变动安全防护设施时，必须经施工负责人同意，并采取相应的可靠措施，作业后应立即恢复。 2、对临边高处作业，必须设置可靠的防护措施并符合下列规定: ?对料台及挑平台周边，雨篷与挑檐边等都必须设施防护栏杆。 ?二层楼面周边，必须在外围架设安全平网一道。 ?分层施工的楼梯口和梯段边，必须安全临时护栏。 ?附墙吊和脚手架等与建筑物通道的两侧边，必须设防护栏杆。 临边防护杆杆件规格及连接要求应符合有关规定。防护栏杆必须自上而下用安全立网封闭，或在栏杆下边设置严密固定的高度不低于18cm的挡脚板或40cm的挡脚笆，板与笆下边距离底面的空隙不应大于10cm，接料平台两侧的栏杆必须自上而下加挂安全立网或满扎竹笆。 3、对于洞口高处作业，必须按下列规定设置防护设施: (1)板与墙的洞口，必须设置牢固的盖板、防护栏杆、安全网或其它防坠落的防护设施。 (2)施工现场通道附近的各类洞口与坑槽处，除设置防护设施与安全标志外，夜间还应设红灯示警。 4、洞口根据具体情况采取防护栏杆、加盖件，张挂安全网等措施时，必须满足下列要求: ?楼板、屋面和平台等面上短边尺寸小于25cm，但大于2.5cm的孔口，必须用坚实的盖板封闭，盖板应能防止挪动移位。 ?楼板面等处边长25,50cm的洞口，可用竹木等作盖板，盖住洞口，盖板须能保持四周搁置均衡，并有固定其位置的措施。 ?边长为50,150cm的洞口，必须设置以扣件扣接钢管而成的网格，并在其上满铺竹笆或脚手板。 ?边长在150cm以上的洞口，四周设防护栏杆，洞口下张设安全平网。 ?对管道井应加设明显标志，如有临时性拆移，需经施工负责人批准，工作完毕后，必须恢复防护设施。 ?墙面等处的竖向洞口，凡落地式的洞口应加装开关式、工具式或固定 23 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 式防护门，门栅网格的间距不应大于15cm，也可采用防护栏杆，下设挡脚板。 5、悬空作业处应有牢靠的立足处，并视具体情况配置防护拦网、栏杆或其它安全设施。悬空作业所用的索具、脚手板、吊笼、吊篮、平台等设备，均需经过技术鉴定或检验后方可使用。 十三、现场消防防火措施 ?水带充水后防骤然折弯，不被油类污染，用后清洗晾干，收藏时应单层卷起，竖放在架上。 ?各种管接口和扪盖应接装灵便，松紧适度，无泄漏，不得与酸、碱等化学品混放，使用时不得摔压。 ?消火栓按室内、室外(地上、地下)的不同要求进行定期检查和及时加注润滑油，消火栓应经常清理，冬季采用防冻措施。 3、料场仓库的防火措施: ?库存物品应分堆贮存编号，对危险物品应加强入库检验，易燃易爆物品应使用不发火的工具设备搬运和装卸。 ?库房内防火设施齐全，应分组布置种类适宜的灭火器，每组不少于2 2个，组间距不大于30m，重点防火区应每25m布置1个灭火器。 ?库房内不得兼做加工、办公或其它用途。 ?库房内严禁使用碘钨灯，电气线路和照明应符合安全规定。 ?易燃易爆物品应保持通风良好，经常检查其温、湿度，防止自燃起火。 ?各种气瓶均应单独设库存放。 4、木工作业棚防火措施: ?作业棚应采用阻燃材料搭设。 ?处于刨花、锯末较多部位的发动机应装设防尘罩。电气设备应密封或采用防爆型。电箱不得堆放物料。 ?防止电线短路、用电设备过载运行或设备漏油和缺油。 ?严禁在作业场所内吸烟、生火、烧饭或点明火取暖。 ?配备足够的灭火器材。 25 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 5、电、气焊作业防火措施: ?焊、割作业点与氧气瓶、电石桶、乙炔发生器的距离不小于10米，与易燃易爆物品的距离不得小于30米。 ?乙炔发生器与氧气瓶之间的距离，在存放时不得小于2米，在使用时不得小于5米。 ?氧气瓶、乙炔发生器等焊割设备上的安全附件完整有效。 ?严格执行“十不烧”规定。 ?作业前应有书面的防火交底，作业时备有灭火器材，作业后清理热源和切断电源、气源。 6、涂(喷)漆作业防火措施: ?作业场所应通风良好，防止空气形成爆炸浓度。采用防爆型电器设备，严禁火源带入。 ?禁止与焊割作业同时或同部位上下交叉进行。 ?接触涂料、稀释剂的工具应采用防火花型。 ?浸有涂料、稀释剂的破布、棉纱、手套和工作服等应及时清除，防止堆放生热自燃。 7、楼内防火措施: ?设置具有足够扬程的高压水泵和室内消火栓。 ?楼层内每层设置4组灭火器材。重点防火部位数量应适当增加。 ?楼层内每层设置4个砂箱，以供紧急灭火备用。 ?设置专职防火监护人员巡回检查，发现火灾隐患，立即采取措施。 ?楼内的防火通道保证畅通，按消防规定，需增设防火墙的地方增设防火墙。 十四、应急预案 1)工地现场急救措施 1、触电急救 首先使触电者迅速脱离电源，然后根据触电者情况进行救护。 ? 伤势不重，神志清醒或曾一度昏迷但已清醒过来的，应使触电者安 26 伤害较重，无知觉、无呼吸，但有心跳，应立即进行人工呼吸，(迅速松开触电者领扣、裤带、清除口腔内杂物，采用口对口或口对鼻吹气法，仰卧压胸法，仰卧压背法，搓臂压胸法等人工呼吸方法)，并立即送入医院。 ? 如呼吸、心跳全无，在救护医生未到之前，应不间断地进行人工呼吸和人工胸外心脏挤压。 2、跌落急救 如果伤口流血，先进行止血，后如果本人不能起来，应立即通知120急救，防止伤到脊椎。 3、伤口 先进行消毒处理，后止血包扎。 4、火灾救护 工地现场一旦发生火灾，不要慌乱，要及时组织灭火，同时挂119电话报警。 ? 油类起火，要用干粉、泡沫灭火器或用麻袋覆盖。 ? 电器类起火，严禁用水或泡沫灭火器灭火，应用干粉灭火器，或干砂进行灭火。 5、中暑急救 发现有中暑病人，应立即将其抬到阴凉通风的地方，且病人平卧，解开衣带，头上敷冷水毛巾，给以清凉饮料，浓茶和解暑药物如:上滴水、人丹，如病人有昏迷现名胜立即将其送附近医院救治。 2)、火灾事故应急预案 施工现场火灾等级可分为A、B两级: A级:火灾现场火势较大，难控制，已产生损失。 27 广福花园B地块1#~13#楼、幼儿园及地下车库安全文明施工组织设计 B级:火灾现场已起火，但火势较小，能控制。 人触电以后，会出现神经麻痹、呼吸中断、心脏停止跳动等征象，外表上呈现昏迷不醒的状态。但不应该认为是死亡，而应该看作是假死，应该迅速而持久地进行抢救。有触电者经四小时或更长时间的人工呼吸而得救的事例。据国外有关经验，除电者在触电后6分钟开始救治者，10%有良好效果;而从触电后12分钟开始救治者，救活的可能性就极小，由此可知，动作迅速是非常重要的。 报警处理程序:发现触电者在无法给予救助的情况下应立即通知施工现场传达室;传达室工作人员在接到报告后;一方面切断现场总电源，一方面以对讲机通知应急小组成员，应急小组急救组应立即赶往触电现场急救，应急小组设备组立即赶行配电间，切断总电源;现场救护由应急小组总指挥统一调度，其它小组成员通知“120”急救。 4)、饮食卫生突发事故应急预案 29 B类:小范围(10人以下)人群事物中毒、一般疾病可以控制，不影响现场生产。 预控目标:防止食物中毒管理目标:杜绝高空坠落事故发生，高处作业监控率达100%。 报警处理程序:A类:发现有疾病者立即通知生活区值班室或施工现场 传达室;值班室及传达室工作人员判断病(疫)情后;一方面拔打急救中 心电话“120”，一方面以对讲机通知应急小组成员病(疫)情，一方面组 织医务人员作救护准备; 安全生产是我国的一项重要政策，也是现代企业管理重要原则之一，做好这项工作对于保证劳动者在生产中的安全和健康，促进现代化企业经济建设具有非常重要的意义，为了搞好安全生产加强对各级施工人员的安全教育，建立各级安全生产责任制，确定安全生产目标，制定安全考核生产制度，使安全生产责、权、利相统一，以实现“预防为主、安全生产”的方针，对项目工程安全生产进行全过程评价和考核。 一、安全生产责任制(参各级安全生产责任制) 二、目标管理内容 1、安全目的;施工现场必须实行安全生产目标管理，工程开工前应制定总的安全管理目标，包括伤亡事故指标、安全达标和文明施工目标以及采取的安全措施。 2、安全管理的实施措施 ?项目经理定期(至少每月一次)召开安全生产会议，了解总结前阶段生产情况布置下阶段工作;?安全教育(根据安全教育计划内容);?安全检查的(安全检查制度);?不安全因素整改，项目部下达整改计划，其完成率不得低于90%，重大事故隐患必须定期整改;?安全控制点管理，制度无漏洞，检查无差错，设备无故障，人员无违章;?安全评比，每季组织安全评比活动，优胜班得优胜流动红旗，并发给1000-5000元的资金。 三、考核内容 根据公司考核办法及分配原则，通过年度安全生产考评和经济奖罚，使目标管理与项目部、班组及个人的经济利益直接挂钩。 1、对项目部的考核:主要是考核项目部在完成一项单位工程后的伤亡情况。如发生应根据考核办法，在经济上对其进行处罚，一般为15000-50000元。 4、对操作工人的考核:主要考核其有否违反从安全管理准则，遵章守纪、文明施工等。若有违反，第一次教育，第二次开除并扣发其当月工资。 二、现场文明施工管理制度 1、本项目要达到“示范工地”。 2、确定领导者:由罗军负责文明施工的管理工作，建立责任制，并抓紧落实。 3、划分责任区域:在划分区域时，应在平面图上标明界限，不要有遗漏，使整个施工现场区域划分责任明确，任何一个角落都有人负责。 4、共同管理:现场文明施工涉及到各个部门或各个工种，大家要齐抓共管，个个动手，才能有良好的成绩。 5、日常管理:文明施工工作贯穿于整个施工过程，加强管理就必须加强日常的管理工作。因此必须建立健全合理的规章制度，对各项工作提出明确的标准和要求，并贯彻于施工过程中去，从而实现经常化。 6、落实奖罚责任制:要严格按制度办，完成任务要表扬，有错误批评还要罚，不论职务，一视同仁。
