FOR SHEATHLESS INSERTION FOR SHEATHED INSERTION PRESSURE MONITORING SET UP The angle of insertion should be 45˚ or less. Make a small incision at the exit of the guidewire. Insert the vessel dilator over the guidewire, tapered end first. Push then twist to firmly attach the one-way valve to the male luer fitting of the IAB catheter. Using the provided 30cc syringe apply a 30cc vacuum. Twist and remove the syringe, keeping the one-way valve attached to the male luer fitting of the IAB catheter. Carefully spread the subcutaneous tissue to facilitate sheathless insertion. Remove the IAB catheter from the T-handle by pulling STRAIGHT out to avoid damaging it. Advance the IAB catheter over the guidewire into the artery in short strokes until correct placement is achieved. Advance the sheath seal as close to the insertion site as possible and secure the IAB catheter to the patient’s leg. Insert the introducer dilator into the sheath hub and secure. Advance the sheath over the guidewire into the artery using a rotary motion. Withdraw the introducer dilator leaving the sheath in place. Remove the IAB catheter from the T-handle by pulling STRAIGHT out to avoid damaging it. Advance the IAB catheter in short strokes through the sheath into the artery until correct placement is achieved. Push the sheath seal into the hub of the sheath and secure the IAB catheter to the patient’s leg. Remove the guidewire and aspirate 3cc of blood from the inner lumen. Perform a manual flush of the inner lumen with 3-5cc of flush solution. Attach a standard arterial fluid pressure monitoring apparatus. Twist and pull to remove the one-way valve from the IAB catheter. Connect the IAB catheter’s male luer fitting to the female luer fitting of the catheter extender.
